目的:探讨细胞周期蛋白(cyclin D1)和Ki67蛋白在胃肠间质瘤(GIST)发生发展中的作用及其临床病理意义。方法:将101例手术切除的GIST组织标本分为低危组、中危组和高危组,应用免疫组织化学SP法检测cyclin D1和Ki67蛋白在GIST组织中的表达情况。结果:低危组、中危组和高危组Ki67蛋白标记指数分别为4.6±1.6、19.8±7.5和38.6±8.2,3组间差异有统计学意义(P<0.05);Ki67蛋白标记指数与核分裂像数目、肿瘤直径有关。cyclin D1蛋白在GIST组织中阳性表达率显著高于瘤旁组织(P<0.05);低危组、中危组与高危组的cyclin D1蛋白表达率分别为36.8%、57.1%和80.0%,3组间差异有统计学意义(P<0.05);cyclin D1表达的阳性率与核分裂像数目有关,但与肿瘤直径及肿瘤坏死无关。结论:cyclin D1和Ki67蛋白的表达情况能够提示GIST的恶性程度,有可能成为判断GIST生物学行为的评价指标。
Objective: To investigate the role of cyclin D1 and Ki67 in the development of gastrointestinal stromal tumors (GISTs) and their clinicopathological significance. Methods: 101 cases of resected GIST tissues were divided into low risk group, moderate risk group and high risk group. The expression of cyclin D1 and Ki67 protein in GIST tissues was detected by immunohistochemical SP method. Results: The Ki67 protein index of low-risk group, intermediate-risk group and high-risk group were 4.6 ± 1.6, 19.8 ± 7.5 and 38.6 ± 8.2, respectively. There was significant difference between 3 groups (P <0.05); Ki67 protein index and mitotic index Like the number of tumor diameter. The positive rates of cyclin D1 protein in GIST tissues were significantly higher than those in adjacent tissues (P <0.05). The protein expression rates of cyclin D1 were 36.8%, 57.1% and 80.0% in low-risk group, middle-risk group and high-risk group respectively The difference between the two groups was statistically significant (P <0.05). The positive rate of cyclin D1 expression was related to the number of mitosis, but not to tumor diameter and tumor necrosis. Conclusions: The expression of cyclin D1 and Ki67 can be used to predict the malignant degree of GIST, which may be used as an index to judge the biological behavior of GIST.
