目的 通过研究胃粘膜中S100~+,HLA-DR~+,CD_4~+和CD_8~+产物的表达情况,以了解DC细胞在胃粘膜免疫中的作用。 方法 胃癌(143例)、慢性萎缩性胃炎(41例)、不典型增生(15例)、肠上皮化生(27例)和正常对照组(10例)共236例,都经内镜和病理诊断明确,采用HE染色、SP免疫酶标抗体组化染色法与图象分析技术相结合,观察胃粘膜中S-100~+,HLA-DR~+,CD_4~+和CD_8~+细胞数量、平均面积和平均吸光度的变化。 结果 胃癌患者胃粘膜中S-100~+(0.6±0.2)μm~2,HLA-DR~+(0.9±0.2)细胞数量明显多于正常胃粘膜(P<0.05);CD_4~+细胞(0.7±0.2)μm~2及其平均吸光度显著减少(P<0.01);CD_8~+细胞数量(1.1±0.3)μm~2、面积及其平均吸光度约有减少,但与正常组比较无显著差异(P>0.05);CD_4~+/CD_8~+比值为0.7,显著低于正常胃粘膜(P<0.01),慢性萎缩性胃炎、肠上皮化生和不典型增生胃粘膜中S-100~+分别为(1.8±0.3)μm~2,(2.2±0.6)μm~2和(2.2±0.5)μm~2,HLA-DR~+分别为(2.4±0.6)μm~2,(3.0±0.8)μm~2和(2.8±0.6)μm~2细胞数量显著增加(P<0.01);CD_8~+细胞数量分别为(1.8±0.2)μm~2,(1.9±0.2)μm~2,(1.8±0.2)μm~2均明显高于正常胃粘膜(P<0.05);CD_4~+/CD_8~+比值分别为0.8,0.6,0.6,均显著低于正常胃粘膜(P<0.05)。 结论 癌前
Objective To investigate the role of DC cells in gastric mucosal immunity by studying the expression of S100~+, HLA-DR~+, CD_4~+ and CD_8~+ products in gastric mucosa. Methods A total of 236 cases of gastric cancer (143 cases), chronic atrophic gastritis (41 cases), atypical hyperplasia (15 cases), intestinal metaplasia (27 cases) and normal control group (10 cases) were examined by endoscopy and pathology. The diagnosis was clear, combined with HE staining, SP immunohistochemistry antibody histochemical staining and image analysis techniques to observe the number of S-100~+, HLA-DR~+, CD_4~+, and CD_8~+ cells in gastric mucosa.
