目的 :探讨乳腺癌在MPMNs中的地位。方法 :回顾性分析 195 8年～ 1997年收治的 398例乳腺癌伴MPMNs。结果 :1)双原发乳腺癌发生率为 2 5 % ,发病年龄 41岁 ,间隔时间 5 4年。 5、10、2 0年生存率分别为86 6 %、5 7 7%和 2 5 8%。 2 )同一乳腺PD伴乳腺癌发病率为 0 9% ,高发年龄为 47 3岁。 5、10、2 0年生存率为 :6 4 7%、5 0 %和 2 0 6 %。 3)乳腺癌伴其他脏器癌瘤的MPMNs发生率为 3 6 % ,高发年龄 49 4岁 ,间隔时间为 8 6年。好发器官为肺、子宫、食管、卵巢和大肠。 4)乳腺癌作为第二、三原发癌的MPMNs发生率 2 3% ,年龄 5 0 1岁 ,间隔时间 7 1年 ;第一原发癌好发器官为 :子宫、大肠、卵巢和食管。结论 :1)乳腺是MPMNs的主要受累器官。2 )乳腺癌伴发MPMNs中易受累的器官为 :女性激素的靶器官 (乳腺、子宫、卵巢 )和乳腺癌放疗区域的器官 (肺和食管 )。 3)正确诊断和治疗可得到较好的治疗效果。
Objective: To explore the role of breast cancer in MPMNs. Methods: Retrospective analysis of 398 cases of breast cancer with MPMNs from 195 to 1997. Results: 1) The incidence of double primary breast cancer was 25%, the age of onset was 41 years, and the interval was 54 years. The 5-, 10-, and 20-year survival rates were 86.6%, 57.7%, and 25.8%, respectively. 2) The incidence of breast cancer with PD in the same breast was 0 9%, and the age of high incidence was 47 3 years. The 5-, 10-, and 20-year survival rates were: 67.7%, 50%, and 20.6%. 3) The incidence of MPMNs in breast cancer associated with other organ cancers was 36%, with a high age of 49 4 years and an interval of 8 6 years. Good organs are the lungs, uterus, esophagus, ovary, and large intestine. 4) The incidence of MPMNs as the second or third primary cancer in breast cancer was 23%. The age was 501 years old and the interval was 71 years. The primary organ of primary breast cancer was: uterus, large intestine, ovary and esophagus. Conclusion : 1) The mammary gland is the main affected organ of MPMNs. 2) Organs susceptible to breast cancer associated with MPMNs are female target organs (mammary glands, uterus, ovaries) and organs (lung and esophagus) in breast cancer radiotherapy areas. 3) The correct diagnosis and treatment can get better therapeutic effect.
