目的研究细支气管肺泡癌对~(18)氟脱氧葡萄糖(FDG)的摄取特点。方法 1998年12月~2004年10月间,35例细支气管肺泡癌患者术前行FDG-正电子发射体层显像(PET)检查。PET结果用目测法与半定量分析方法判读。半定量分析方法中计算标准摄取值(SUV)。结果所有肿瘤都被FDG-PET探测到,目测法相应部位FDG不同程度浓聚。半定量分析细支气管肺泡癌组织的FDG摄取(SUVmax and SUVmean)高于相应的正常肺组织(SUVlung)(P<0.001),SUVmax、SUVmean和SUVlung分别为3.14±1.65、2.40±1.34 和0.38±0.08。细支气管肺泡癌SUV与肿瘤大小正相关(P<0.05)。21例(21/35,60.0%)SUVmean与15例(15/35,42.9%)SU Vmax低于2.5。这21例肿瘤全部被目测法与半定量分析判断为良性。结论 (1)细支气管肺泡癌组织的FDG摄取高于正常肺组织。(2)肿瘤FDG摄取与肿瘤大小有关。(3)很多细支气管肺泡癌病例 FDG-PET检查可呈现假阴性。
Objective To study the uptake of ~ (18) fluorodeoxyglucose (FDG) by bronchioloalveolar carcinoma. Methods From December 1998 to October 2004, 35 patients with bronchioloalveolar carcinoma were examined by FDG-PET before operation. PET results with visual methods and semi-quantitative analysis of interpretation. Semi-quantitative analysis method to calculate the standard intake value (SUV). Results All the tumors were detected by FDG-PET, and the corresponding parts of the visual field were concentrated to varying degrees. Semiquantitative analysis of bronchioloalveolar carcinoma tissue showed higher FDG uptake (SUVmax and SUVmean) than the corresponding normal lung tissue (SUVlung) (P <0.001), SUVmax, SUVmean and SUVlung were 3.14 ± 1.65, 2.40 ± 1.34 and 0.38 ± 0.08 . SUV of bronchioloalveolar carcinoma was positively correlated with tumor size (P <0.05). SU Vmax was lower than 2.5 in 21 cases (21/35, 60.0%) SUVmean and 15 cases (15/35, 42.9%). All 21 tumors were visually and semi-quantitatively determined as benign. Conclusions (1) FDG uptake in bronchioloalveolar carcinoma was higher than that in normal lung tissue. (2) tumor FDG uptake and tumor size. (3) FDG-PET in many bronchioloalveolar carcinoma cases can show false negatives.
