目的分析血尿酸(SUA)在高血压伴靶器官损害患者中的水平变化及临床意义。方法选择2013年1月—2014年12月我院收治的1 320例原发性高血压患者作为病例组,根据病例组有无靶器官损害分为无靶器官损害组和有靶器官损害组,选择同时期门诊健康体检者100例作为对照组,比较各组对象SUA水平。结果 1 320例患者中伴有靶器官损害1 091例(82.65%);病例组SUA水平显著高于对照组(P<0.05);有靶器官损害组SUA水平高于无靶器官损害组(P<0.05);3个及以上靶器官损害患者SUA水平显著高于1个和2个靶器官损害患者(P<0.05);SUA与高血压患者年龄、胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL-C)、尿素氮(BUN)、肌酐(Cr)、空腹血糖(FBG)、左心室质量指数(LVMI)、收缩压(SBP)、舒张压(DBP)呈显著正相关性(r>0,P<0.05),与高密度脂蛋白(HDL-C)呈显著负相关性(r<0,P<0.05);多因素Logistic回归分析结果显示,高血压病程(OR=5.169)、SBP(OR=4.323)、DBP(OR=4.215)、靶器官损害(OR=7.145)、LVMI(OR=5.991)、血肌酐(Scr)(OR=5.772)对患者SUA有明显影响(P<0.05)。结论高血压患者SUA水平普遍升高,且SUA水平与高血压靶器官损害及患者糖脂代谢紊乱有密切关系,血压(BP)、Scr及LDL-C水平与患者SUA明显相关。
Objective To analyze the changes and clinical significance of serum uric acid (SUA) in patients with hypertension and target organ damage. Methods A total of 1,320 patients with essential hypertension treated in our hospital from January 2013 to December 2014 were selected as case group. According to the presence or absence of target organ damage in case group, the patients were divided into the group without target organ damage and the target organ damage group, Select the same period outpatient health examination of 100 cases as a control group, the comparison of SUA levels in each group. Results 1 320 cases were associated with target organ damage in 1 091 cases (82.65%), SUA level in case group was significantly higher than that in control group (P <0.05), SUA level in target organ damage group was higher than that in non-target organ damage group <0.05). The levels of SUA in three or more target organ damage patients were significantly higher than those in one or two target organ damage patients (P <0.05). SUA was associated with age, cholesterol (TC), triglyceride (TG) , LDL-C, BUN, Cr, FBG, LVMI, DBP and DBP were significantly positive (R> 0, P <0.05) and HDL-C (r <0, P <0.05). The multivariate Logistic regression analysis showed that the duration of hypertension (OR = 5.169), SBP (OR = 4.323), DBP (OR = 4.215), target organ damage (OR = 7.145), LVMI (OR = 5.991) and serum creatinine (P <0.05). Conclusions The level of SUA is generally elevated in hypertensive patients. SUA level is closely related to the target organ damage in patients with hypertension and the disturbance of glucose and lipid metabolism. The levels of blood pressure (BP), Scr and LDL-C are significantly correlated with SUA.
