本文研究再障和白血病在环磷酰胺(CY)单独处理或CY加全身放疗(TBI)后骨髓移植病人的卵巢功能。方法骨髓移植后已生存1年而无复发的女性病人187例。43例为再障,急、慢白血病144例。移植时平均年龄25(13~49)岁。移植后平均4(1~15)年。所有病人在移植前月经周期正常。移植前再障病人接受200mg/kgCY,白血病人接受200mg/kgCY加9.2~15.75Gy总量的射线。从移植后1~15年的不同时期,分别评价性腺的功能,测定促卵泡激素(FSH)、黄体生成素(LH)、雌二醇(E_2)含量。结果再障病人43例在移植后出现绝经。36例在移植后3~42月恢复正常月经周期,FSH、LH、E_2水平正常。在移植时年龄13~25岁的病人,移植后均有卵巢功能恢复,多次激素水平测定保持正常。有6例8次受孕,出生7名正常婴儿,1例人工流产。在移植时年龄26~38岁的16例中,9例在移植后6~18月恢复正常月经周期,激素水平正常。但4例恢复是暂时的,2~6年后出现绝经期症状,同时FSH、LH水平升高,E_2水平降低。3例病人4次受孕,出生1名正常婴儿,2例自然流产,1例人工流产。16例中其余7例没有恢复卵巢功能。白血病病人143例移植后头3年内均出现停经,
This article investigates the ovarian function of patients with aplastic anemia and leukemia undergoing bone marrow transplantation following cyclophosphamide (CY) treatment alone or CY plus systemic radiotherapy (TBI). Methods A total of 187 female patients survived 1 year without recurrence after bone marrow transplantation. 43 cases of aplastic anemia, acute and chronic leukemia in 144 cases. The average age of transplant 25 (13 to 49) years old. After transplantation an average of 4 (1 ~ 15) years. All patients had normal menstrual cycles before transplantation. Patients with aplastic anemia prior to transplantation receive 200 mg / kg of CY and leukemic patients receive a total dose of 200 mg / kg of CY plus 9.2 to 15.75 Gy. The gonadal function was evaluated at different time points from 1 to 15 years after transplantation. The levels of follicle stimulating hormone (FSH), luteinizing hormone (LH) and estradiol (E2) were determined. Results Aplastic anemia occurred in 43 patients with aplastic anemia after transplantation. 36 cases returned to normal menstrual cycle 3 ~ 42 months after transplantation, FSH, LH, E_2 levels were normal. At the time of transplantation, patients aged 13 to 25 years had ovarian function recovery after transplantation and the determination of multiple hormone levels remained normal. There were 6 cases of 8 conceptions, 7 normal babies born and 1 case of induced abortion. Of the 16 patients aged 26 to 38 years old, 9 returned to normal menstrual cycles from 6 to 18 months after transplantation, with normal hormone levels. But 4 cases of recovery is temporary, 2 to 6 years after menopause symptoms, while FSH, LH levels, E 2 levels decreased. 3 patients were pregnant 4 times, a normal child was born, 2 cases of spontaneous abortion, 1 case of induced abortion.
