目的:运用速度向量成像超声(VVI)技术评价在常规疗法基础上加用温胆片治疗对脑梗死恢复期颈动脉粥样硬化患者颈动脉弹性等的影响。方法:选取40例脑梗死恢复期合并颈动脉粥样硬化的患者,随机分为2组,对照组予营养脑细胞、改善血液循环、抗凝等常规治疗,治疗组予常规治疗加服温胆片,2组疗程均为3月。于治疗前、治疗3月后用VVI软件自动识别和跟踪颈动脉血管内膜,自动测量各节段最大切向应变(S_(max))及最大切向应变率(SR_(max))、收缩期峰值血流速度(PSV)、阻力指数(RI)、内膜-中膜厚度(IMT)。结果:治疗后,治疗组IMT、RI值均较治疗前下降,PSV值较治疗前升高,差异均有统计学意义(P<0.05)。对照组3项指标值治疗前后比较,差异均无统计学意义(P>0.05)。治疗组3项指标与对照组比较,差异均有统计学意义(P<0.05)。2组VVI技术检测结果:治疗组颈内动脉内膜速度向量大小、方向比较趋向一致,对照组颈内动脉血流方向紊乱、不同步;治疗组颈内动脉各个节段的S_(max)、SR_(max)均高于对照组,运动幅度也较大。结论:在常规疗法基础上加用温胆片治疗脑梗死恢复期合并颈动脉粥样硬化患者,可改善颈动脉血供及颈动脉弹性。VVI技术能有效显示颈动脉内壁弹性情况,较为准确地评估临床疗效。
OBJECTIVE: To evaluate the effect of bile duct tablet on carotid artery elasticity in patients with carotid atherosclerosis during recovery from cerebral infarction by using velocity vector imaging (VVI) technique. Methods: Forty patients with concomitant carotid atherosclerosis during convalescence of cerebral infarction were randomly divided into two groups. The control group was fed with nutrient brain cells to improve blood circulation and anticoagulant therapy. The patients in the treatment group received conventional therapy plus warm gall bladder Tablets, two groups of treatment are March. Before treatment, VIV software was used to automatically identify and track the intima of carotid arteries after 3 months of treatment. The maximum tangential strain (S max) and maximum tangential strain (SR max) of each segment were automatically measured and contracted Periodic peak blood flow velocity (PSV), resistance index (RI), intima-media thickness (IMT). Results: After treatment, the IMT and RI values of the treatment group decreased compared with those before treatment, and the PSV value was significantly higher than that before treatment (P <0.05). There were no significant differences in the three indexes in the control group before and after treatment (P> 0.05). Three indicators of the treatment group compared with the control group, the difference was statistically significant (P <0.05). The results of VVI technique in two groups showed that the velocity and direction of internal carotid artery in the treatment group tended to be the same, and the direction of internal carotid artery flow was disordered and asynchronous in the control group. The S max, SR_ (max) were higher than the control group, exercise amplitude is also larger. Conclusion: Based on routine therapy, WENBA tablet can improve carotid artery blood supply and carotid artery elasticity in patients with carotid atherosclerosis during recovery from cerebral infarction. VVI technology can effectively show the elasticity of the carotid artery wall, a more accurate assessment of clinical efficacy.
