目的探讨经乳晕缘切口乳腺良性肿瘤切除术的安全性及应用价值。方法对在门诊手术室行乳腺良性肿瘤切除术患者资料进行回顾分析和随访,比较经乳晕缘切口组(A组66例)和经肿瘤原位切口组(B组75例)的手术时间、术后并发症、切口瘢痕评分(VSS评分)及满意度。分析乳晕缘切口患者术后乳晕区感觉异常与切口长度及肿瘤部位的相关性。结果A组的手术时间较B组有显著延长(P<0.01)。两组中均有1例术后切口感染,均无术后血肿的发生。A组VSS评分和患者的满意度评价优于B组,差异有统计学意义(P<0.01)。在A组中,乳晕区感觉异常与肿瘤部位有关(P=0.02),尤以外下和内下象限的肿瘤切除术后易出现感觉异常,但未发现与切口长度有关(P=0.54)。结论乳腺良性肿瘤经乳晕缘切口手术安全、可靠,肿瘤暴露良好,术后瘢痕不显著,美体效果好,适于推广应用。但对于肿瘤位于外下和内下象限的,术后易出现乳晕区感觉异常,应予重视。
Objective To investigate the safety and application value of benign tumor resection by breast margin incision. Methods Retrospective analysis and follow-up of patients undergoing benign breast tumor resection in outpatient operating room were performed. The operative time and technique of the patients who underwent surgical resection of the marginal rim (66 cases in group A) and in situ incision (75 cases in group B) Postoperative complications, incisional scar score (VSS score) and satisfaction. To analyze the relationship between postoperative heliostate sensory abnormalities and incision length and tumor location in patients with posterior rim incision. Results The operation time of group A was significantly longer than that of group B (P <0.01). One case of postoperative incision infection in both groups, no postoperative hematoma occurred. The score of VSS in group A and the evaluation of patient satisfaction were better than those in group B, the difference was statistically significant (P <0.01). In group A, the areola area was associated with abnormality in the tumor site (P = 0.02). In particular, sensory abnormalities were evident after tumor resection of the outer and inferior quadrant, but no correlation was found with the incision length (P = 0.54). Conclusion Surgical treatment of breast benign tumor is safe and reliable, the tumor is well exposed, the postoperative scar is not significant, and the body effect is good. It is suitable for popularization and application. However, the tumor is located outside the lower quadrant and the lower quadrant, postoperative prone to areolar abnormalities, should be taken seriously.
