目的分析血清视黄醇结合蛋白(RBP)、胱抑素C(CysC)、血管内皮生长因子(VEGF)在糖尿病肾病(DN)中的变化及意义。方法将124例2型糖尿病(T2DM)患者根据24h尿微量白蛋白排泄率(UAER)随机分为正常白蛋白尿(NA)组35例,微量白蛋白尿(MA)组45例和临床白蛋白尿(CP)组44例,另选取35例健康体检者为对照组,记录各组临床资料,测定各组生化指标,应用ELISA法测定各组血清RBP、CysC、VEGF水平并比较。结果 NA组、MA组、CP组BMI、腰围(WC)、空腹血糖(FPG)、餐后2h血糖(2h FBG)、糖化血红蛋白(Hb Alc)、餐前C肽(CP)、餐后2h C肽(2h CP)、胰岛抵抗指数(HOMA-IR)、血肌酐(Cr)、尿素氮(BUN)、RBP、CysC、VEGF水平显著高于对照组(P<0.05)。与NA组比,MA组、CP组BMI、WC、FPG、2h FBG、Hb Alc、CP、2h CP、HOMA-IR、Cr、BUN、RBP、CysC、VEGF水平较高(P<0.05),而CP组血清RBP、CysC、VEGF水平高于MA组。经Spearman单因素分析,RBP与UAER、BUN、Cr呈正相关(P<0.05),CysC与UAER、BNU、Cr呈正相关(P<0.05),VEGF与Hb Alc、HOMA-IR、RBP、CysC呈正相关(P<0.05)。结论血清RBP、CysC、VEGF水平与DN发生有密切的关系,血清RBP、CysC、VEGF水平可作为DN早期诊断指标。
Objective To analyze the changes of serum retinol binding protein (RBP), cystatin C (CysC) and vascular endothelial growth factor (VEGF) in diabetic nephropathy (DN). Methods A total of 124 patients with type 2 diabetes mellitus (T2DM) were randomly divided into normal albuminuria (NA) group (n = 35), microalbuminuria (MA) group and clinical albumin Serum levels of RBP, CysC and VEGF were measured by ELISA in 44 cases of urinary (CP) group and 35 healthy subjects as control group. The clinical data of each group were recorded and the biochemical indexes of each group were recorded. Results The levels of BMI, WC, FPG, 2h FBG, Hb Alc, CP in the NA group, MA group and CP group after 2h C The levels of peptide (2h CP), insulin resistance index (HOMA-IR), serum creatinine (Cr), blood urea nitrogen (BUN), RBP, CysC and VEGF were significantly higher than those of the control group (P <0.05). Compared with NA group, BMI, WC, FPG, FBG, Hb Alc, CP, 2h CP, HOMA-IR, Cr, BUN, RBP, CysC and VEGF in MA group and CP group were higher than those in NA group Serum levels of RBP, CysC and VEGF in CP group were higher than those in MA group. There was a positive correlation between RBP and UAER, BUN and Cr (P <0.05), CysC and UAER, BNU and Cr (P <0.05), VEGF and Hb Alc, HOMA-IR, RBP and CysC by Spearman’s univariate analysis (P <0.05). Conclusions Serum levels of RBP, CysC and VEGF are closely related to the occurrence of DN. Serum levels of RBP, CysC and VEGF can be used as indicators for the early diagnosis of DN.
