目的对比荧光定量聚合酶链式反应法(FQ-PCR)及染色涂片法在脑脊液结核杆菌检测中的应用价值。方法结核性脑膜炎(TBM)疑似患者320例,临床医师在无菌操作下抽取患者脑脊液送至实验室进行FQ-PCR法与染色涂片法检测,观察检测结果。结果 320例TBM疑似患者共确诊45例。FQ-PCR法检测的阳性率为15.63%,明显高于染色涂片法的6.25%,差异具有统计学意义(P<0.05);染色涂片法检测的特异度为92.31%、灵敏度为44.44%,均低于FQ-PCR方法的98.21%、90.91%,差异具有统计学意义(P<0.05)。结论相较于染色涂片法,FQ-PCR法具有更高的特异度及灵敏度,且操作快速、简便,在脑脊液结核杆菌的检测中具有重要的应用价值。
Objective To compare the diagnostic value of fluorescent quantitative polymerase chain reaction (FQ-PCR) and staining smear in detection of Mycobacterium tuberculosis in cerebrospinal fluid. Methods A total of 320 suspected tuberculous meningitis (TBM) patients were enrolled. Clinicians aseptically collected CSF from patients and sent them to the laboratory for FQ-PCR and staining. The results were observed. Results A total of 320 suspected TBM patients were diagnosed in 45 cases. The positive rate of FQ-PCR was 15.63%, which was significantly higher than that of staining method (6.25%), the difference was statistically significant (P <0.05). The specificity of staining was 92.31% and the sensitivity was 44.44% , Both lower than 98.21% and 90.91% of the FQ-PCR method, the difference was statistically significant (P <0.05). Conclusion Compared with the staining smear method, FQ-PCR method has a higher specificity and sensitivity, and the operation is quick and easy, and has an important application value in the detection of Mycobacterium tuberculosis.
