目的应用经胸实时三维超声心动图(RT3D-TTE)研究扩张型心肌病(DCM)患者二尖瓣的结构及功能变化,探讨RT3D-TTE评价二尖瓣结构及功能的临床价值,以期为DCM患者二尖瓣反流的临床治疗提供参考依据。方法 30例DCM患者作为DCM组,35例健康志愿者作为对照组,两组均行超声心动图检查,比较两组超声心动图参数、二尖瓣参数。结果 DCM组左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)均大于对照组(P<0.05);左室射血分数(LVEF)为(23.25±7.98)%,小于对照组的(62.25±4.10)%(P<0.05)。DCM组二尖瓣环前后直径(AP)、瓣环前外侧至后内侧直径(AL-PM)、球度指数(SPI)、幕状区高度(TH)、瓣环二维面积(AA2D)、瓣环三维面积(AA3D)均大于对照组,瓣环高度(AH)、瓣环位移(ADMax)均小于对照组(P<0.05)。结论经胸实时三维超声心动图可定量的评价DCM患者二尖瓣形态及功能,为DCM患者二尖瓣反流的临床治疗提供参考依据。
Objective To investigate the changes of mitral valve structure and function in patients with dilated cardiomyopathy (DCM) by real-time three-dimensional transthoracic echocardiography (RT3D-TTE) and to explore the clinical value of RT3D-TTE in evaluating the structure and function of mitral valve The clinical treatment of patients with mitral regurgitation provide a reference. Methods Thirty patients with DCM were selected as DCM group and 35 healthy volunteers as control group. Echocardiography was performed in both groups. Echocardiographic parameters and mitral valve parameters were compared between the two groups. Results The LVEDV and LVESV in DCM group were significantly higher than those in control group (P <0.05). LVEF was (62.25 ± 4.10)% (P <0.05). The anterior and posterior diameter (AP), AL-PM, SPI, TH, and AA2D of the mitral annulus in DCM group were significantly higher than those in DCM group The three-dimensional area of the annulus (AA3D) was larger than that of the control group. The height of the annulus and the axis of the annulus were smaller than those of the control group (P <0.05). Conclusion The real-time three-dimensional transthoracic echocardiography can quantitatively evaluate the morphology and function of mitral valve in patients with DCM, and provide a reference for the clinical treatment of DCM patients with mitral regurgitation.
