目的采用神经导航辅助脑室镜技术手术治疗儿童侧脑室蛛网膜囊肿,探讨两者结合应用对开展小儿神经内镜手术的意义。方法对7例患儿,男5例,女2例,运用神经导航制定手术计划,设计理想的手术切口和入路,实时、动态地监测内镜的操作轨迹。内镜下行囊肿开窗术4例,切除术3例。结果全部病例均获得良好的治疗效果,切除手术中次全切除2例,部分切除1例。术后无菌性脑膜炎2例,无出血、感染及神经功能缺失等并发症。结论结合神经导航技术,可以弥补病理情况下脑室内局部解剖变异导致内镜操作中定位困难的缺陷,使脑室内复杂手术的精确性更高,手术安全有效。神经导航辅助脑室镜技术在儿童微创神经外科手术中具有广阔的发展前景。
Objective To investigate the clinical significance of neurovascular assisted intraventricular surgery in the treatment of children with arachnoid cysts. Methods 7 cases of children, 5 males and 2 females were enrolled in this study. Nerve navigation was used to make surgical plans and to design ideal surgical incision and approach to monitor the operation path of endoscope in real time and dynamically. Endoscopic cyst in 4 cases of fenestration, resection in 3 cases. Results All the cases received good curative effect. In subtotal resection, subtotal resection was performed in 2 cases and in partial resection in 1 case. 2 cases of aseptic meningitis after surgery, no bleeding, infection and neurological deficits and other complications. Conclusions Combining with neural navigation technology, it can make up for the defect that the local anatomical variation of the ventricle in the pathological situation leads to the difficult localization in the endoscopic operation, and make the accuracy of the complex surgery in the intraventricular compartment higher and the operation safe and effective. Neuronavigation assisted ventricular mirror technology in children with minimally invasive neurosurgery surgery has broad prospects for development.
