目的 :探讨苯氧乙酸类降血脂药非诺贝特 (fenofibrate)对实验性糖尿病大鼠肾脏的保护作用。方法 :将实验用SD大鼠随机分为正常对照组 (C组 ) ,糖尿病组 (D组 ) ,非诺贝特治疗组 (F组 )。第 4、6周分别随机取各组大鼠一半检测血糖、血甘油三酯、血胆固醇、血肌酐、尿白蛋白及肾脏肥大指数。应用免疫组织化学方法检测肾组织转化生长因子 - β1 (TGF - β1 )、层粘连蛋白 (LN) ,IV型胶原蛋白的表达。用计算机图象分析系统测定肾小球平均体积(MGV)、肾小球平均面积 (MGA)。结果 :非诺贝特治疗组血甘油三酯、血胆固醇、尿白蛋白排泄率均低于糖尿病组 (P<0 0 5 ,P <0 0 1 )。第 6周时 ,F组TGF - β1 、LN、IV型胶原蛋白表达低于D组 (P <0 0 5)。结论 :非诺贝特对糖尿病肾脏有部分保护作用 ,其机制可能部分通过降血脂、下调糖尿病大鼠TGF - β1 的表达减少细胞外基质的积聚。
Objective: To investigate the protective effect of fenofibrate, a phenoxyacetate hypolipidemic drug, on the kidneys of experimental diabetic rats. Methods: Experimental SD rats were randomly divided into normal control group (group C), diabetic group (group D) and fenofibrate treatment group (group F). At the 4th and 6th week, half of rats in each group were randomly selected to test blood glucose, triglyceride, blood cholesterol, serum creatinine, urinary albumin and renal hypertrophy index. Immunohistochemistry was used to detect the expression of transforming growth factor - β1 (TGF - β1), laminin (LN) and collagen type Ⅳ in renal tissue. The mean glomerular volume (MGV) and mean glomerular area (MGA) were determined by a computerized image analysis system. Results: The rate of excretion of blood triglyceride, blood cholesterol and urinary albumin in fenofibrate treatment group was lower than that in diabetic group (P <0.05, P <0.01). At the 6th week, the expression of TGF - β1, LN, and type IV collagen in group F was lower than that in group D (P <0.05). CONCLUSION: Fenofibrate has some protective effects on diabetic nephropathy, and its mechanism may partly reduce the accumulation of extracellular matrix by lowering blood lipids and down - regulating the expression of TGF - β1 in diabetic rats.
