目的:为了探讨糖尿病微血管与大血管并发症之间的内在联系。方法:采用多普勒超声诊断仪检测133例中老年2型糖尿病(T2DM)患者颈动脉内中膜厚度(IMT),探讨IMT的改变与糖尿病视网膜病变(DR)的关系。结果:本研究发现,T2DM患者颈动脉平均IMT与糖尿病视网膜病变的发生有密切关系,IMT增厚组的DR总患病率以及增殖性DR(PDR)患病率显著高于IMT正常组。糖尿病病程、糖化血红蛋白、颈动脉内中膜厚度与视网膜病变显著相关;而年龄、收缩压和糖化血红蛋白与颈动脉斑块显著相关。结论:说明糖尿病大血管及微血管病变发病关系密切而又机制存在差异,预防策略也应相应有所不同。
Objective: To explore the intrinsic relationship between diabetic microvascular and macrovascular complications. Methods: The carotid intima - media thickness (IMT) in 133 middle - aged and old patients with type 2 diabetes mellitus (T2DM) was examined by Doppler sonography to explore the relationship between the changes of IMT and diabetic retinopathy (DR). Results: This study found that, in patients with T2DM, the average carotid IMT and diabetic retinopathy are closely related to the occurrence of IMT thickening group DR prevalence and proliferative DR (PDR) prevalence was significantly higher than the IMT normal group. Diabetes mellitus, glycosylated hemoglobin, and carotid intima-media thickness were significantly associated with retinopathy; age, systolic blood pressure, and HbA1c were significantly associated with carotid plaques. Conclusion: It shows that the pathogenesis of diabetic macroangiopathy and microvascular disease are closely related and the mechanism is different, and the preventive strategies should be correspondingly different.
