目的 探讨脉压与炎性因子C -反应蛋白之间性别差异的原因及其意义。方法我们对江苏太仓市区 4 6 3名 (男性 2 2 4人 ,女性 2 39人 )受试者分 2次随访共测量血压 10次 ,用日本产OlympusAU2 70 0全自动生化分析仪测量全项生化指标 ,用德国产BNprospec免疫比浊仪测量超敏C -反应蛋白 (hsCRP) ,进行单因素和多因素回归分析。结果在对年龄、体重指数、吸烟饮酒史、服药史校正前后 ,女性的脉压总是与C -反应蛋白有显著的相关性 (P <0 0 0 2 ) ,而男性则无相关性 (P >0 10 )。在女性 ,血清C -反应蛋白浓度升高 1倍 ,脉压增加1 94mmHg。经相同方法校正后 ,男、女性的收缩压、舒张压与C -反应蛋白之间均未达到统计学差异 (P >0 0 5 )。用四分位数间距将男、女性血清CRP各自分层分析 ,相同方法校正后 ,四分位数与一分位数比较 ,女性的脉压有 7 6mmHg差别 ( 95 %可信区间为 3 5~ 11 7mmHg ,P =0 0 0 0 3) ;而男性未发现有显著性差别 (P >0 0 5 )。 结论 慢性炎性因子C -反应蛋白对我国汉族女性脉压增宽起重要作用
Objective To investigate the causes and significance of gender differences in pulse pressure and inflammatory factor C - reactive protein. Methods A total of 463 subjects (224 males and 239 females) in Taicang City, Jiangsu Province were divided into two follow-up visits. Total blood pressure was measured 10 times. All the patients were tested with Olympus AU2 70 0 automatic biochemical analyzer Biochemical indicators, using German BNprospec immunoturbidimetric measurement of high sensitivity C - reactive protein (hsCRP), univariate and multivariate regression analysis. Results Before and after adjustment for age, BMI, smoking and drinking history and medication history, the pulse pressure in women always had a significant correlation with C - reactive protein (P <0.00 2), but no correlation with male (P > 0 10). In women, the serum C - reactive protein concentration increased 1 time, pulse pressure increased 1 94mmHg. After adjustment by the same method, there was no significant difference between systolic blood pressure, diastolic blood pressure and C - reactive protein in males and females (P> 0.05). Male and female serum CRP levels were stratified by quartiles of quartiles, and after adjustment for the same method, the quartile was compared with the one-quantile, with a difference of 76 mmHg in female pulse pressure (95% confidence interval was 35 ~ 11 7mmHg, P = 0 0 0 0 3). However, no significant difference was found in males (P> 0.05). Conclusion Chronic inflammatory cytokine C - reactive protein plays an important role in the pulse pressure widening in Chinese Han women
