目的了解河北任丘老年居民胰岛素抵抗患病率及相关危险因素,为进一步开展慢性病管理、治疗及健康教育工作提供依据。方法于2012年以整群随机抽样法选择任丘市年龄60~70岁常住居民为研究对象,共5 010例,男性2 163例,女性2 847例,调查内容包括健康调查问卷、体格检查、实验室检测。并用logistic回归方法分析胰岛素抵抗的相关危险因素。结果胰岛素抵抗检出率为24.3%(1 218例),男性为17.2%(371例),女性为29.8%(847例)。多因素logistic回归分析结果显示,居住在城区、糖尿病、甘油三酯(TG)、腰围、体质指数是男、女性胰岛素抵抗的共同独立危险因素(男性OR值分别为1.650、1.610、1.226、1.053和1.067,女性OR值分别为1.851、2.247、1.152、1.040和1.089),HDL-C是男、女性胰岛素抵抗的保护因素(男、女性OR值分别为0.534、0.403)。尿酸升高是女性胰岛素抵抗的独立危险因素(OR=1.003)。结论河北任丘老年居民胰岛素抵抗的检出率高,积极推进健康生活方式,使体重、腰围、血脂和尿酸控制在合理范围内,可改善胰岛素抵抗的发生,使本地区慢性非传染性疾病的流行在源头上得以遏制。
Objective To investigate the prevalence of insulin resistance and related risk factors among elderly residents in Renqiu, Hebei Province, and to provide evidences for further management, treatment and health education of chronic diseases. Methods A total of 5 010 residents aged 60-70 years old in Renqiu City were enrolled in this study. They were 2 163 males and 2 847 females in 2012. The survey included questionnaires of health, physical examination, Laboratory testing. Logistic regression was used to analyze the risk factors associated with insulin resistance. Results The detection rate of insulin resistance was 24.3% (1 218 cases), 17.2% (371 cases) in men and 29.8% (847 cases) in women. Multivariate logistic regression analysis showed that living in urban areas, diabetes, triglyceride (TG), waist circumference and body mass index were common independent risk factors of insulin resistance in men and women (male OR values were 1.650,1.610,1.226,1.053 and 1.067, OR = 1.851,2.247,1.152,1.040 and 1.089 respectively). HDL-C was the protective factor of male and female insulin resistance (OR = 0.534 and 0.403, respectively). Elevated uric acid is an independent risk factor for female insulin resistance (OR = 1.003). Conclusion The elderly residents in Renqiu, Hebei Province, have a high detection rate of insulin resistance and actively promote a healthy lifestyle, so that their body weight, waist circumference, blood lipids and uric acid can be controlled within a reasonable range, which can improve the occurrence of insulin resistance and make chronic non-communicable diseases The epidemic can be stopped at the source.
