目的 评估结核分枝杆菌RNA恒温扩增实时荧光检测技术(simultaneous amplification and testing for Mycobacterium tuberculosis,SAT-TB)检测痰液标本对肺结核的快速诊断价值.方法 检索中国学术期刊全文数据库(CNKI)、万方数据库、维普数据库、PubMed、ScienceDirect和Cochrane图书馆等数据库,收集有关探讨SAT-TB检测痰液对肺结核诊断价值的所有文献,检索起止时间为建库至2016年12月.拟定入选标准、排除标准,2名研究者分别独立进行文献资料提取及文献质量评估,采用软件Meta-DiSc 1.4进行Meta分析.结果 共检索出168篇文献,依据入选标准和排除标准,纳入了9篇文献进行分析,包含了5137例临床患者痰标本,所有标本均进行了SAT-TB检测和痰结核分枝杆菌培养.9篇文献质量采用QUADAS-2评价表评价,文献偏倚风险主要为患者选择方面.Meta分析结果显示,SAT-TB法检测痰液诊断肺结核的汇总敏感度为87.0% (95%CI:86.0%~89.0%),特异度为90.0%(95%CI:88.0%~91.0%),阳性似然比(positive likelihood ratio,PLR)为6.91(95%CI:4.04~11.84),阴性似然比(negative likelihood ratio,NLR)为0.09(95%CI:0.06~0.15).结论 SAT-TB检测痰液对肺结核快速诊断具有较高的敏感度和特异度,值得临床进一步推广应用.“,”Objective To evaluate the rapid diagnostic value of simultaneous amplification and testing for Mycobacterium tuberculosis (SAT-TB) from sputum in patients with pulmonary tuberculosis.Methods All studies involved the diagnostic value of SAT-TB for detection Mycobacterium tuberculosis RNA from sputum samples for pulmonary tuberculosis were searched in CNKI,Wanfang,VIP,PubMed,ScienceDirect and Cochrane Library databases from establishment to December 2016.The inclusion and exclusion criteria for screening literatures was formulated.Two researchers independently fetched the data from articles and assessed their quality.Meta-DiSc (version 1.4)was used for Meta-analysis.Results The literatures search resulted in 168 abstracts identified and 9 studies with full text were recruited for analysis including 5137 sputa samples from patients with pulmonary tuberculosis according to the inclusion and exclusion criteria,and all samples were detected Mycobacterium tuberculosis RNA by SAT-TB and MTB by culture.The quality of 9 studies was assessed with QUADAS-2 and the main bias risk was case selection.In all studies,the pooled sensitivity,pooled specificity,positive likelihood ratio and negative likelihood ratio of SAT-TB were 87.0% (95%CI:86.0%-89.0%),90.0% (95%CI:88.0%-91.0%),6.91 (95%CI:4.04-11.84) and 0.09 (95 % CI:0.06-0.15),respectively.Conclusion The rapid diagnostic value of SAT-TB for Mycobacterium tuberculosis RNA from sputum samples for pulmonary tuberculosis is very high with high sensitivity and specificity.SAT-TB is worth for clinical application in the future.
