目的探讨矽肺发病机理中的细胞免疫损伤及其对矽肺患者肺功能的影响。方法应用免疫组化法观察30例矽肺患者及29例健康正常人CD3、CD4、CD8的分布状况,并研究CD3、CD4、CD8细胞亚群与肺功能的相关性。结果各期矽肺患者CD3、CD4、CD8细胞较正常组均有明显改变,CD4减低(P<0.05),CD4/CD8减低,两组之间差异具有统计学意义(P<0.01);CD3的改变与肺弥散功能成正相关(r=0.68)。结论各亚型淋巴细胞参与了矽肺患者免疫损伤,其细胞免疫损伤直接导致肺弥散功能的改变,符合矽肺的病理生理改变。
Objective To investigate the cellular immune injury in the pathogenesis of silicosis and its effect on lung function in patients with silicosis. Methods The distribution of CD3, CD4 and CD8 in 30 silicosis patients and 29 healthy controls were observed by immunohistochemistry. The correlation between CD3, CD4, CD8 cell subsets and pulmonary function was also studied. Results The levels of CD3, CD4 and CD8 in all stages of silicosis were significantly lower than those in the normal control group (P <0.05), while the levels of CD4 / CD8 were decreased in both groups (P <0.01) And lung diffusion function is positively correlated (r = 0.68). Conclusion The subtypes of lymphocytes are involved in the immunological injury of silicosis patients. The cellular immunity damage directly leads to the change of pulmonary diffusing function, which accords with the pathophysiological changes of silicosis.
