目的研究血浆中可溶型B7-H3(sB7-H3)与乙型肝炎病毒相关性肝癌(HBV-HCC)的关系。方法选择在解放军第302医院住院治疗的HBV-HCC患者23例及健康体检者(HC)15例,采用ELISA检测外周血浆中sB7-H3含量;设计sB7-H3基因特异性引物,采用相对实时定量PCR(qRT-PCR)方法检测外周血单个核细胞(PBMC)、癌和癌旁组织中sB7-H3 mRNA的表达;采用免疫组织化学染色检测癌和癌旁组织中的膜型B7-H3(mB7-H3)表达。结果 HBV-HCC患者血浆中sB7-H3含量显著高于健康对照组(P<0.05),但HCC患者和健康对照组PBMC中sB7-H3的mRNA表达水平无显著差异(P>0.05);而肝癌切除术后HBV-HCC患者血浆中sB7-H3含量显著下降(P<0.05),同时HBV-HCC患者癌和癌旁组织中表达的sB7-H3 mRNA比其PBMC显著升高,分别可达到PBMC中sB7-H3 mRNA表达量的60倍和1 800倍;癌旁组织中的sB7-H3 mRNA水平高于自身癌组织近30倍;癌旁组织中mB7-H3表达显著高于癌组织。结论肝癌及癌旁组织是外周血中sB7-H3的主要来源之一,sB7-H3可作为HBV-HCC的辅助诊断标志物。
Objective To study the relationship between plasma soluble B7-H3 (sB7-H3) and hepatitis B virus associated hepatocellular carcinoma (HBV-HCC). Methods Twenty-three HBV-HCC patients and 15 healthy volunteers (HC) were enrolled in the 302 Hospital of People’s Liberation Army. The levels of sB7-H3 in peripheral blood plasma were determined by ELISA. The sB7-H3 gene specific primers were designed and used for real- PCR was used to detect the expression of sB7-H3 mRNA in peripheral blood mononuclear cells (PBMCs), cancer tissues and adjacent non-cancerous tissues. Immunohistochemical staining was used to detect the expression of membrane type B7-H3 (mB7 -H3) expression. Results The plasma levels of sB7-H3 in HBV-HCC patients were significantly higher than those in healthy controls (P <0.05). However, the mRNA levels of sB7-H3 in PBMC of HCC patients and healthy controls were not significantly different (P> 0.05) The level of sB7-H3 in the plasma of patients with HBV-HCC was significantly lower after resection (P <0.05), and the expression of sB7-H3 mRNA in cancer tissues and adjacent tissues of HBV-HCC patients was significantly higher than PBMC sB7-H3 mRNA expression was 60-fold and 180-fold higher than that of adjacent tissues. The level of sB7-H3 mRNA in paracancerous tissues was nearly 30 times higher than that in auto-cancerous tissues. The expression of mB7-H3 in paracancerous tissues was significantly higher than that in cancerous tissues. Conclusion Hepatocarcinoma and its adjacent tissues are one of the main sources of sB7-H3 in peripheral blood, and sB7-H3 can be used as a diagnostic marker for HBV-HCC.
