目的分析精神科药物引起的肝功能实验室检查异常与心肌酶谱增高的相关性。方法选取精神科药物引起的肝功能实验室检查异常患者112例为甲组,检测其心肌酶谱;选取精神科住院心肌酶谱异常患者60例为乙组,检测其肝功能。对2组检测结果进行相关性分析。结果精神科药物引起的肝功能实验室检查异常与心肌酶谱天门冬氨酸氨基转移酶(AST)、α羟基丁酸脱氢酶(HBDH)、乳酸脱氢酶(LDH)及磷酸肌酸激酶(CK)增高呈正相关(P<0.01),与磷酸肌酸激酶同工酶(CKMB)无相关性(P>0.05)。结论心肌酶谱增高是由于药物损伤肝细胞引起,若无心电图异常和其他临床资料提示心肌严重受损,可按药物性肝损伤处理,减少精神科药物引起的肝损伤的发生。
Objective To analyze the correlation between abnormalities of liver function laboratory tests and elevated myocardial enzymes caused by psychiatric drugs. Methods A total of 112 patients with abnormal hepatic function laboratory tests were selected as group A and their myocardial enzymes were determined. Another 60 patients with abnormal myocardial enzymes in psychiatric hospital were selected as Group B and the liver function was tested. Two groups of test results for correlation analysis. Results The abnormalities of liver function laboratory tests induced by psychiatric drugs were associated with myocardial enzymes such as aspartate aminotransferase (AST), alpha hydroxybutyrate dehydrogenase (HBDH), lactate dehydrogenase (LDH) and creatine phosphokinase (CK) increased (P <0.01), but no correlation with creatine kinase MB (CKMB) (P> 0.05). Conclusions The increase of myocardial enzymes is caused by the injury of hepatocytes caused by drugs. Without ECG abnormality and other clinical data suggest serious myocardial damage, drug-induced liver injury may be treated to reduce the occurrence of liver damage caused by psychiatric drugs.
