目的探讨Klotho基因在残余肾模型大鼠肾脏的表达变化及淫羊藿、黄芪、大黄复方对其表达的影响。方法应用5/6肾切除建立残余肾大鼠模型,随机分为对照组、假手术组、模型组、骨化三醇组、中药复方2.25 g/ml剂量组、中药复方4.5 g/ml剂量组,给予相应药物灌胃8周。给药前、后检测大鼠尿素氨(blood urea nitrogen,BUN)、血肌酐(serum creatinine,SCr)、内生肌酐清除率(creatinine clearance rate,Ccr)、血红蛋白(hemoglobin,HGB),用药结束取肾组织行病理检查,Real-time PCR、Western blotting检测Klotho表达。结果中药复方2.25 g/ml、中药复方4.5 g/ml组大鼠BUN、Scr与骨化三醇组、模型组比较明显降低;中药复方2.25 g/ml、中药复方4.5 g/ml组Ccr、HGB与模型组比较明显升高;模型组大鼠肾脏病理表现肾小球局灶节段硬化、小管萎缩塌陷,中药复方干预后病理改变减轻;模型组、骨化三醇组大鼠肾组织Klotho基因、Klotho蛋白表达显著低于正常组和假手术组,中药复方2.25 g/ml、中药复方4.5 g/ml组Klotho基因、Klotho蛋白表达明显高于模型组。结论 Klotho基因在残余肾模型大鼠肾脏的表达降低,淫羊藿、黄芪、大黄复方能上调残余肾组织Klotho基因表达、减轻病理改变、减缓肾衰竭进程。
