目的:运用logistic回归分析方法,探讨影响急性痛风性关节炎疗效因素,为提高治疗急性痛风性关节炎疗效提供科学依据。方法:152例确诊的急性痛风性关节炎病例,来自8个临床研究分中心,随机分成中医治疗组78例和西医治疗组74例。中医治疗方案包括基础治疗、痛风颗粒加新癀片;西医治疗方案为基础治疗和秋水仙碱,收集患者治疗前后的临床表现。疗效评价参照《中药新药临床研究指导原则》(试行)2002年版。数据分析主要采用Logistic回归。结果:中医治疗组中,关节功能评分、脉弦、小便短赤与疗效相关,其中关节功能评分、脉弦与疗效呈正相关,小便短赤呈负相关。西医治疗组中,舌质红、脉弦、关节活动受限评分、便干结与疗效相关,其中舌质红、脉弦与疗效呈正相关,关节活动受限评分、便干结呈负相关。将回归分析结果中显示的相关性症状列入治疗的适应症,分析后的结果表明:中、西医治疗组的疗效均有所提高。结论:一些因素影响中、西医治疗急性痛风性关节炎疗效,为了提高临床治疗效果,应加强此方面的进一步研究。
Objective: To investigate the curative effect of acute gouty arthritis by logistic regression analysis and provide scientific basis for improving curative effect of treating acute gouty arthritis. METHODS: A total of 152 confirmed cases of acute gouty arthritis from eight clinical study centers were randomly divided into a Chinese medicine treatment group (78 cases) and a western medicine treatment group (74 cases). Chinese medicine treatment programs include basic treatment, gout particles plus new tablets; Western treatment programs based on the treatment and colchicine, to collect patients before and after treatment of clinical manifestations. Evaluation of the efficacy of reference “Chinese medicine clinical research guidelines” (for trial implementation) 2002 edition. Data analysis mainly uses Logistic regression. Results: In the TCM treatment group, the score of joint function, pulse chord and shortness of urine were correlated with the curative effect. The score of joint function, the relationship between pulse string and curative effect, and the short stool of red urine were negatively correlated. Western medicine treatment group, red tongue, pulse string, limited joint activity score, then dry knot and curative effect, including red tongue, pulse string and curative effect was positively related to joint activity limit score, then dry knot was negatively correlated. The results of regression analysis showed that the symptoms of treatment included in the indications for treatment, the analysis of the results showed that: Chinese and Western medicine treatment group have improved the efficacy. Conclusion: Some factors affect the efficacy of Western medicine in the treatment of acute gouty arthritis. In order to improve the effect of clinical treatment, further research in this area should be strengthened.
