目的探讨彩色多普勒超声诊断子宫圆韧带囊肿的价值。方法回顾性分析11例经手术及病理证实为子宫圆韧带囊肿患者的超声图像。结果彩色多普勒超声检查发现腹股沟区囊性包块,位于股总动、静脉内侧,包块大小1.6 cm×1.2 cm×1.3 cm~4.7 cm×3.1 cm×3.8 cm,形态规整,边界清晰,纵切呈椭圆形,横切呈扁圆形,囊壁光滑,9例暗区透声好,后方回声增强,2例暗区内见高回声光带及细小点状回声。彩色多普勒血流显像(CDFI)显示,包块内部未见血流信号,周边局部可见少许血流信号。当挤压探头或嘱患者做valsalva动作,体位由卧位转为站立位时,包块的大小、形态及内部回声未见明显改变。术中所见:2例误诊病例为腹股沟斜疝,术中见囊内较多纤维分隔,8例包块内为淡黄色清亮液体,1例包块内为血性液体。结论临床或超声医师对子宫圆韧带囊肿的认识不足是误诊的主要原因之一,其解剖位置又与腹股沟疝等疾病不易区分。彩色多普勒超声能够较直观分辨囊肿与周围组织,特别是血管的解剖关系,图像清晰,定位准确,对该病能做出准确的诊断及鉴别诊断。
Objective To investigate the value of color Doppler ultrasound in diagnosis of uterine round ligament cyst. Methods A retrospective analysis of 11 cases of surgically and pathologically confirmed uterine round ligament cyst in patients with ultrasound images. Results Color Doppler ultrasound examination revealed cystic mass in the inguinal region located in the medial part of the common femoral vein and vein. The mass size was 1.6 cm × 1.2 cm × 1.3 cm ~ 4.7 cm × 3.1 cm × 3.8 cm. The shape of the mass was clear, Slitting oval, cross-oblate, smooth wall, 9 cases of dark areas through the sound good, enhanced echo at the back, dark areas within 2 cases to see the hyperechoic band and small punctate echo. Color Doppler flow imaging (CDFI) showed no signs of blood flow inside the mass, peripheral blood flow can be seen a little signal. When squeezing the probe or instruct patient to do valsalva action, position from the supine to standing position, the mass of the mass, shape and internal echo no significant change. Intraoperative findings: 2 cases of misdiagnosed cases of inguinal hernia, intraoperative see more fibrous septum, 8 cases of light yellow clear liquid within the mass, 1 case of bloody mass within the mass. Conclusion Clinical or sonographer’s lack of understanding of the uterine round ligament cyst is one of the main causes of misdiagnosis. The anatomical location is not easy to distinguish with inguinal hernia and other diseases. Color Doppler ultrasound can more intuitively distinguish the cyst and the surrounding tissue, especially the anatomy of blood vessels, clear images, accurate positioning, the disease can make accurate diagnosis and differential diagnosis.
