目的:探讨精氨酸对围术期剖宫产患者免疫功能影响及临床意义。方法:回顾性分析广西区人民医院2012年10月~2013年4月收治入院的90例无妊娠合并症及并发症的剖宫产患者临床资料,随机分为3组,每组30例,对照组术后常规补液、预防感染治疗,实验组1在对照组常规治疗的基础上术前1天及术后6天静脉补充注射用盐酸精氨酸0.4 g/kg/天,实验组2在对照组常规治疗的基础上术前1天及术后6天静脉补充注射用盐酸精氨酸0.8 g/kg/天,分别于术前、手术后第3天、第7天上午空腹抽血检测白蛋白、IgG、IgM、超敏C反应蛋白(CRF)、淋巴细胞CD3+、CD4+、CD4+/CD8+水平。结果:3组患者术后第3天的白细胞及中性粒细胞总数明显升高,到第7天下降,但仍未恢复到术前水平,3组之间差异无统计学意义(P>0.05);淋巴细胞术后第3天明显减少,第7天回升,也未恢复到术前水平,实验1、2组较对照组恢复良好,之间有显著性差异(P<0.05),但实验1、2组之间差异无统计学意义(P>0.05):3组患者治疗后IgG、IgA、IgM、C3、C4水平明显下降,尤其在术后第3天最明显,术后第7天不同程度得到恢复:术后实验1组与实验2组免疫功能恢复均较对照组好,但两组之间差异无统计学意义(P>0.05):3组患者治疗后T淋巴细胞总数及各亚群均明显减少,以术后第3天达到最低点,术后第7天基本恢复到术前水平;实验1、2组T淋巴细胞总数及各亚群的减少程度均较对照组为轻,CD4+/CD8+阢,值也较对照组早恢复到术前水平;实验1组术后7天的CD4+/CD8+水平较实验2组低,且差异有统计学意义(P<0.05);但两组之间CD3+、CD4+、CD8+等水平差异无统计学意义(P>0.05)。结论:精氨酸能改善围术期剖宫产患者免疫功能,减轻术后发生感染的风险。

The levels of IgG, IgA, IgM, C3 and C4 in the three groups were significantly decreased after treatment, especially on the third day after the operation and on the seventh day after the operation There was no significant difference between the two groups (P> 0.05). The total number of T lymphocytes and the percentage of T lymphocytes in each group after treatment Subpopulations were significantly reduced to reach the lowest point after 3 days after surgery, and basically returned to preoperative levels on the 7th day after surgery; the total number of T lymphocytes in groups 1 and 2 and the degree of reduction of each subgroup were lighter than those in the control group , CD4 + / CD8 + 阢, also returned to the preoperative level as early as the control group. Conclusion: Arginine can improve the immune function of cesarean section patients during perioperative period and reduce the risk of postoperative infection.
