心脏再同步化治疗(cardiac resynchronization therapy,CRT)是治疗充血性心力衰竭(congestive heart failure,CHF)的新方法,它借助于起搏技术使严重的房室传导阻滞或心室内传导障碍患者恢复原有的心脏收缩同步状态,其治疗效果更接近于生理循环状态,因而有助于改善患者的血流动力学状态,并具有抗心律失常作用。现行CRT适应证标准以QRS>120ms选择治疗患者,但因机械收缩不同步与电活动不同步并非完全等同,故临床实践中有必要寻找能更准确评价心脏收缩不同步状态的诊断方法来选择CRT治疗。实时三维超声心动图(real-time three-dimensional echocardiography,RT-3DE)是新近发展起来的新的超声诊断技术,运用RT-3DE实时显示心脏立体解剖结构并定量评价心室同步性,是选择、监控、引导和评价心室同步化治疗的有效方法。
