本文报道1例典型的系统性红斑狼疮,初期的抗核抗体效价增高,当发生严重的肾功能衰竭时,此血清学指标延迟恢复。患者女性,20岁,患肾病综合征伴蝴蝶形皮疹。红斑狼疮细胞阳性,血浆抗核因子(ANF)效价为1/160,脱氧核糖核酸结合抗体(anti-ds DNA)为99%(正常0～20%)。用强的松和硫唑嘌呤治疗,皮疹痊愈后肾病综合征仍未愈,ANF及anti-ds DNA仍呈强阳性。4个月后伴发癫痫与急性少尿性肾功能衰竭,ANF和狼疮细胞反复呈阴性,同时anti-dsDNA正常(18%),血清C_3、C_4浓度降低,血中还可查到免疫复合体。用大剂量的肾上腺皮质酮、环
This article reports a case of a typical case of systemic lupus erythematosus, initial anti-nuclear antibody titer increased, when serious renal failure, the delay in the serological recovery. Female patient, 20 years old, suffering from nephrotic syndrome with butterfly rash. Lupus erythematosus cells were positive with an ANF titer of 1/160 and 99% (normal 0-20%) of anti-ds DNA. With prednisone and azathioprine treatment, nephrotic syndrome after the rash recovered healed, ANF and anti-ds DNA was still strongly positive.
