对32例手术病理证实的肺癌和肺结核病人(其中鳞状上皮癌16例,腺癌8例,小细胞肺癌4例,肺转移癌2例,肺结核2例),进行99mTc-TF SPECT显像和CT检查,将结果直接与手术结果对比,来评价肺和纵膈淋巴结病变。结果在肺癌原发灶,99mTc-TF SPECT显像具有较高的灵敏度(93%, 28/30),但2例肺结核病灶也显示99mTc-TF的摄取增高。有5例手术证实伴有肿瘤中心坏死的肺癌病灶,其99mTc-TF SPECT显像示肿瘤中心部位放射性缺损。在纵膈淋巴结转移,99mTC-TF SPECT显像的特异性和灵敏度(88, 87%)优于CT扫描(59%, 67%)。可见99mTc-TF SPECT显像在评价肺癌原发灶和纵膈淋巴结转移中,特别是后者是一种准确和有效的方法。
Thirty-two patients with lung cancer and tuberculosis confirmed by pathology (including 16 cases of squamous cell carcinoma, 8 cases of adenocarcinoma, 4 cases of small cell lung cancer, 2 cases of metastatic lung cancer, and 2 cases of pulmonary tuberculosis) were performed with 99mTc-TF SPECT imaging and CT examinations were performed to compare the results directly with the results of the surgery to evaluate lung and mediastinal lymph node lesions. Results In primary lung cancer, 99mTc-TF SPECT imaging had higher sensitivity (93%, 28/30), but 2 cases of pulmonary tuberculosis also showed increased uptake of 99mTc-TF. There were 5 cases of lung cancer lesions with necrosis of the tumor centers confirmed by surgery. The 99mTc-TF SPECT imaging showed radioactive defects in the center of the tumor. In the mediastinal lymph node metastases, the specificity and sensitivity of the 99mTC-TF SPECT imaging (88, 87%) was superior to that of the CT scan (59%, 67%). It can be seen that 99mTc-TF SPECT imaging is an accurate and effective method for evaluating primary lung cancer and mediastinal lymph node metastasis, especially the latter.
