目的通过药师对医嘱的审核及干预,分析了解首都医科大学附属北京佑安医院住院药房口服用药医嘱的合理使用情况,促进医院更合理的使用口服药品。方法通过HIS系统选取该院2013年9、10、11月3个月的口服药品长期医嘱并进行实时审核,同时对不合理或不合格医嘱通过电话告知的方式进行干预,对审核结果和干预情况进行统计分析。结果共审核医嘱227 504条医嘱,其中不合理医嘱247条,占全部医嘱的0.109%。主要包括用法用量不合理、重复用药与医嘱重复及配伍禁忌。干预并被采纳医嘱共137条,采纳率为55.47%。结论药师通过对医嘱的审核,提高了患者在口服药品使用中的安全性、合理性及有效性。
Objective To assess the rational use of oral medication prescribed by inpatient pharmacies in Beijing You’an Hospital affiliated to Capital Medical University through the review and intervention of pharmacists on doctor’s orders so as to promote the rational use of oral medicines by hospitals. Methods The HIS system was used to select the long-term prescriptions of oral medicines for the three months of September, October and November 2013 in 2013 and carry out real-time examination. At the same time, interviews were conducted by means of telephone informs of unreasonable or unqualified medical orders. The audit results and interventions conduct statistical analysis. Results A total of 227 504 medical orders were reviewed, of which 247 were unreasonable medical orders, accounting for 0.109% of all medical orders. Including the amount of unreasonable usage, repeat medication and medical advice repeat and incompatibility. A total of 137 interventions were accepted and accepted, with an adoption rate of 55.47%. Conclusion The pharmacist’s examination of the doctor’s advice improves the safety, rationality and effectiveness of the patient in the use of oral medicines.
