1 事故经过 1998年4月10日上午10时,安徽省岳西县河图变电所在维修高压线路时,发生人身触电事故。4月9日,河图电管站余某与河图变电所供电班李某联系,决定10日维修中南至杨坪地段10kV线路。由于该线路联接处跌落保险A相损坏,是用铝线直接导通的,两人决定在河图变电站隔离开关处停电。4月10日8时,在未开工作票也未通知调度室的情况下,余某、汪某等人只挂了“禁止合闸,线路有人工作”的标示牌,便自行断电维修。9时,葫芦河电站赵某为不影响本站发电,主动要求解开中南至杨坪10kV联接处跌落保险并得到河图变电所李某同意。但赵某拉开三相保险时,未注意到保险上端的直通导线。李某未经现场查验,也未通知有关人员,即将河图电站隔离开关合上,致使维修现场的张某、汪某、葛某等人触电受伤。 2 事故原因分析
