目的探讨99mTcO4-涎腺动态显像对干燥综合征(SS)的诊断价值。方法采用MarconiFx810双探头SPECT仪对30名正常志愿者和34例临床确诊的SS患者进行99mTcO4-涎腺动态显像,利用感兴趣区技术生成双侧涎腺时间—放射性曲线,计算摄取指数(UI)、排泌指数(EI)等功能参数,并将SS患者各项功能参数与唇黏膜病理学分级进行相关性分析。结果SS患者涎腺UI、EI显著降低,与对照组比较差异有统计学意义(P<0.01)。腮腺及颌下腺摄取功能指标与病理分级有良好的相关性(r值分别为-0.311,-0.321;P值为0.01,0.008)。腮腺EI值与病理分级无统计意义的相关性,Ⅱ ̄Ⅲ级EI值相近,颌下腺EI值与病理分级无明显相关性,且与正常组有部分交叉。结论99mTcO4-涎腺显像定量分析对SS诊断的灵敏度高,对病情评估、疗效观察、预后判断等均具有重要意义。该方法操作简便、无创、重复性好,值得临床推广应用。
Objective To investigate the diagnostic value of 99mTcO4-salivary gland dynamic imaging in patients with Sjogren’s syndrome (SS). Methods The dynamic imaging of 99mTcO4-salivary gland was performed on 30 normal volunteers and 34 clinically diagnosed SS patients using the Marconi Fx810 dual-probe SPECT. The temporal and radiological curves of bilateral salivary glands were generated by using the region-of-interest technique. The intake index ), Excretion index (EI) and other functional parameters, and the SS patients with various functional parameters and lip mucosa pathological grade correlation analysis. Results The salivary gland UI and EI in SS patients were significantly lower than those in control group (P <0.01). Parotid gland and submandibular gland function index and pathological grading have a good correlation (r values were -0.311, -0.321; P value of 0.01,0.008). Parotid EI value and pathological grade no statistically significant correlation, Ⅱ ~ Ⅲ EI values are similar, submandibular gland EI value and pathological grade no significant correlation, and some with the normal group cross. Conclusion 99mTcO4-salivary gland quantitative analysis of the high sensitivity of SS diagnosis, evaluation of the disease, the efficacy of observation, the prognosis of judgment is of great significance. The method is simple, non-invasive, reproducible, and worthy of clinical promotion and application.
