目的探讨氮端脑钠肽(NT-proBNP)在早产儿动脉导管未闭中的诊断价值。方法选择2011年6月至2012年12月我科收治的存在血流动力学改变动脉导管未闭的早产儿20例为病例组。通气无动脉导管未闭的早产儿20例为对照组。用酶联免疫荧光法分别检测两组早产儿第1天、第3天和第6天的血浆NT-proBNP水平。采用受试者操作特性(ROC)曲线评价诊断价值。结果第3天病例组的血浆NT-proBNP水平显著高于对照组,差异有统计学意义(P<0.05)。第3天NT-proBNP的ROC曲线下面积(AUC)为0.892,临界值(cut-off值)2479pmol/l,敏感性为85%,特异性为85%。结论血浆NT-proBNP水平的检测可作为早产儿有血流动力学改变的动脉导管未闭早诊断良好的辅助指标。
Objective To investigate the diagnostic value of NT-proBNP in the patent ductus arteriosus of premature infants. Methods From June 2011 to December 2012, 20 cases of preterm infants with hemodynamic changes in patent ductus arteriosus admitted to our department were selected as the case group. Ventilation without patent ductus arteriosus in 20 cases of preterm children as control group. Plasma NT-proBNP levels were measured on day 1, day 3 and day 6 of preterm infants by enzyme-linked immunosorbent assay. Subjects were evaluated for diagnostic value using the ROC curve. Results On the third day, the plasma NT-proBNP level in the case group was significantly higher than that in the control group (P <0.05). The area under the ROC curve (AUC) of NT-proBNP on day 3 was 0.892, the cut-off value was 2479 pmol / l, the sensitivity was 85%, and the specificity was 85%. Conclusion The detection of plasma NT-proBNP levels may be a good adjunct to early diagnosis of patent ductus arteriosus with hemodynamic changes in preterm infants.
