目的探讨慢性肾衰竭维持性血液透析患者中低血压事件与血糖的相关性。方法随机选择我院2007年1月-2011年1月需进行维持性血液透析的慢性肾衰竭患者120例,于透析0、2、4h监测血糖和血压,对血透中出现低血压事件者立刻停止超滤,并给予50%葡萄糖注射液40ml静脉注射。结果随着透析时间的延长,血糖逐渐降低,平均动脉压均逐渐下降,所有患者共透析2160次,出现低血糖的有360例次,发生率16.67%;出现低血压事件294例次,发生率为13.61%,相关分析表明血糖与血压间具有显著正相关性(r=0.763,P<0.05)。部分发生低血压事件患者补充50%葡萄糖后血压回升。结论慢性肾衰竭患者血液透析中出现低血压事件的重要危险因素之一是低血糖。
Objective To investigate the association between hypotension and blood glucose in maintenance hemodialysis patients with chronic renal failure. Methods A total of 120 patients with chronic renal failure requiring maintenance hemodialysis were randomly selected from January 2007 to January 2011 in our hospital. Blood glucose and blood pressure were monitored at 0, 2 and 4 hours after dialysis. Stop the ultrafiltration and give 40ml intravenous injection of 50% glucose injection. Results With the extension of dialysis time, blood glucose gradually decreased and mean arterial pressure decreased gradually. All patients were dialyzed 2,160 times with hypoglycemia 360 times, the incidence rate was 16.67%. There were 294 cases of hypotensive events, the incidence rate Was 13.61%. Correlation analysis showed that there was a significant positive correlation between blood glucose and blood pressure (r = 0.763, P <0.05). Some patients with hypotension after 50% glucose supplement blood pressure rise. Conclusions One of the major risk factors for hypotension in hemodialysis patients with chronic renal failure is hypoglycemia.
