长期以来人们为防治青光眼做了大量的研究与探讨,发现青光眼不仅与眼压有关,而且与眼内血管的微循环状态也有关.本文采用王砚耕提出的公式(王砚耕.血压与眼压联合诱发青光眼的生物力学机制.眼科新进展1983;3(3)146.)对40例青光眼患者和20例正常人的血压、眼压及眼动脉灌注压(下面简称灌注压)与青光眼的关系及临床意义作一探讨.1 临床资料与方法选择近两年我科就诊有视野检查的,就诊前未用任何降眼压药物的40例青光眼患者.其中女31例,男9例,年龄最大76岁,最小32岁.急性闭角型青光眼34例(55只眼),慢性闭角型青光眼4例(7只眼),开角型青光眼2例(3只眼),另外3只眼因角膜上皮脱落未能测眼压,12只眼是未发病眼.将患者实测的血压、眼压(修氏眼压计所测)代入“0.8×舒张压+0.926-1.33×眼压”公式,算出患者的灌注压(kPa).按照成人标准舒张压10.664kPa,正常人眼压的上限2.739kPa代入上述公式,得出正常的灌注压下临界值为5.812kPa.低于5.812kPa为低灌注压.对照组20例(40只眼),其中男10例,女10例,年龄最大68岁,最小29岁.血压、眼压测定均在正常范围,视野、房角、眼底检查排除青光眼.按上述公式计算灌注压作对照.
