目的讨论经颅多普勒超声(TCD)在脑动静脉畸形的临床应用价值即对患者的初筛。方法用TCD对几年来就诊患者中发现的8例脑动静脉畸形患者进行总结。用4 MHz连续波和2 MHz探头分别检测颈部和颅内各大血管,同时进行压颈试验和二氧化碳反应试验及脑血流自定调节功能检测。结果用TCD检查后发现了供血动脉高流速低搏动指数的频谱和AVM对脑血流的自动调节功能和二氧化碳反应性降低是动静脉畸形比较特异的指标。结论目前的脑动脉畸形患者日趋增多,用TCD检查快捷方便,对患者无损害、经济实惠,对进一步的脑血管造影进行初筛。
Objective To discuss the clinical value of transcranial Doppler ultrasound (TCD) in cerebral arteriovenous malformations. Methods TCD was used to summarize 8 cases of cerebral arteriovenous malformations found in several years of visiting patients. The 4 MHz continuous wave and 2 MHz probe were used to detect the major blood vessels in the neck and the cranium respectively. Meanwhile, neck compression test, carbon dioxide response test and self-regulation of cerebral blood flow were performed. Results After TCD examination, it was found that low frequency pulsatile index of arterial blood flow and automatic regulation of AVM on cerebral blood flow and carbon dioxide reactivity were more specific indicators of arteriovenous malformations. Conclusions The current patients with cerebral arterial malformations are increasing in number. TCD examination is quick and convenient, without any harm to the patients and is economical. The preliminary screening of further cerebral angiography is carried out.
