目的 :检测乳腺良、恶性病变中 Bcl- 2表达及意义。方法 :对 1 1 1例乳腺良、恶性病变的石蜡切片标本进行免疫组织化学染色。结果 :乳腺良、恶性病变中乳腺导管内乳头状瘤病的 Bcl- 2阳性率为 2 5.0 %。与无表达的乳腺纤维腺病和纤维腺瘤差异显著 ( P<0 .0 5)。乳腺癌组织中 Bcl- 2高表达 ( 68.6% ) ,这种表达与肿瘤组织学分级和淋巴结转移呈负相关 ( P<0 .0 1 ) ,与激素受体水平和5年生存期呈正相关 ,与肿物大小无关。结论 :Bcl- 2表达的导管内乳头状瘤病容易癌变。Bcl- 2过表达的乳腺癌 ,生存期长 ,预后好 ,Bcl- 2有可能作为判定乳腺导管内乳头状瘤病早期癌变及乳腺癌预后的有效指标。
Objective: To detect the expression of Bcl-2 in benign and malignant breast lesions and its significance. Methods: Immunohistochemical staining of 111 paraffin - embedded specimens of benign and malignant breast lesions was performed. Results: The positive rate of Bcl-2 in breast intraductal papillomatosis in benign and malignant breast lesions was 25.0%. There was a significant difference between non-expressed breast fibroadenosis and fibroadenoma (P <0.05). Bcl-2 was highly expressed in breast cancer (68.6%). This expression was negatively correlated with tumor histological grade and lymph node metastasis (P <0.01), positively correlated with hormone receptor level and 5-year survival rate Has nothing to do with the size of the tumor. Conclusion: The Bcl-2 expression of intraductal papilloma is easily carcinogenic. Bcl-2 overexpression of breast cancer, long survival and good prognosis, Bcl-2 may be used as a judgment of breast ductal papilloma early carcinogenesis and prognosis of breast cancer effective indicators.
