目的利用判别分析法建立判别值,评价其在新生儿先天性肾上腺皮质功能增生症(CAH)筛查中的应用。方法对广西新生儿疾病筛查中心2009年10月~2015年3月的筛查资料进行回顾性分析,根据孕周(周)、体重(g)及17-羟孕酮(17-OHP)水平(nmol/L),采用非标准化Fisher判别法建立判别公式及阳性判别值。结果建立的判别公式为:Z=(-0.225·x1)+(-0.000 285·x2)+0.54·x3+9.188,阳性判别值为27.12。确诊的16例CAH患儿经判别分析法判别,结果与诊断结果 100%相符。真阳性率从1.02%上升至2.17%。结论 Z值判别分析法可以减少新生儿CAH筛查中的假阳性率。
Objective To establish discriminant value by discriminant analysis and evaluate its application in the screening of neonatal congenital adrenal cortical hyperplasia (CAH). Methods The screening data of Guangxi Neonatal Screening Center from October 2009 to March 2015 were retrospectively analyzed. According to gestational age (weeks), body weight (g) and 17-OHP level (nmol / L), using non-standardized Fisher discriminant to establish discriminant formula and positive discriminant value. The discriminant formula was established as follows: Z = (- 0.225 · x1) + (- 0.000 285 · x2) + 0.54 · x3 + 9.188, and the positive discriminant value was 27.12. The diagnosis of 16 cases of children with CAH by discriminant analysis of discrimination, the results and the diagnosis of 100%. True positive rate increased from 1.02% to 2.17%. Conclusion Z-value discriminant analysis can reduce the false positive rate of neonatal CAH screening.
