目的探讨急性肺损伤(ALI)大鼠肺表面活性物质相关蛋白A(SP-A)mRNA表达与支气管肺泡灌洗液(BALF)中肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)水平的相关性。方法大肠埃希杆菌[O111B4,(4.4~5.6)×1012CFU·L-1]3mL·kg-1气管注射制作大鼠肺内源性ALI模型(模型组),对照组气管注射3mL·kg-19g·L-1盐水。气管注射大肠埃希杆菌后24h行动脉血气分析,ELISA法检测BALF中TNF-α、IL-8水平,反转录-PCR法检测肺组织SP-A mRNA表达,并与TNF-α、IL-8行相关性分析。结果 1.模型组动脉血氧分压[pa(O2):(7.71±1.27)kPa]明显低于对照组[(13.03±0.92)kPa](P=0.000)。2.模型组BALF中TNF-α、IL-8水平[(409.03±82.77)ng·L-1,(80.76±25.03)ng·L-1]均明显高于对照组[(108.92±17.35)ng·L-1,(32.54±12.10)ng·L-1](Pa=0.000)。3.肺组织SP-AmRNA表达(0.31±0.15)明显低于对照组(0.66±0.16)(P=0.000)。4.SP-A mRNA表达与TNF-α水平呈显著负相关(r=-0.626,P=0.01),与IL-8水平无显著相关性(r=-0.207,P=0.441)。结论炎性因子TNF-α、IL-8在ALI发病机制中起重要作用,ALI肺组织SP-A mRNA表达明显下降,且与BALF中TNF-α水平呈显著负相关。
Objective To investigate the expression of lung surfactant associated protein A (SP-A) and the levels of tumor necrosis factor-α (TNF-α) and interleukin-8 in bronchoalveolar lavage fluid (BALF) (IL-8) levels of the correlation. Methods Rat lung endogenous ALI model (model group) was established by injecting 3 mL · kg-1 of Escherichia coli [O111B4 (4.4 ~ 5.6) × 1012CFU · L-1] into the trachea and the control group was injected with 3 mL · kg- L-1 brine. The levels of TNF-α and IL-8 in BALF were detected by ELISA. The expression of SP-A mRNA in lung tissue was detected by reverse transcription-PCR. The levels of TNF-α and IL- 8-line correlation analysis. PaO2: (7.71 ± 1.27) kPa] in the model group was significantly lower than that in the control group [(13.03 ± 0.92) kPa] (P = 0.000). The levels of TNF-α and IL-8 in BALF in model group [(409.03 ± 82.77) ng · L -1, (80.76 ± 25.03) ng · L -1] were significantly higher than those in control group [(108.92 ± 17.35) ng · L-1, (32.54 ± 12.10) ng · L-1] (Pa = 0.000). The lung SP-AmRNA expression (0.31 ± 0.15) was significantly lower than the control group (0.66 ± 0.16) (P = 0.000). There was a significant negative correlation between the expression of SP-A mRNA and TNF-α (r = -0.626, P = 0.01), but not with the level of IL-8 (r = -0.207, P = 0.441). Conclusions The inflammatory cytokines TNF-α and IL-8 play an important role in the pathogenesis of ALI. The expression of SP-A mRNA in ALI lung tissue is significantly decreased and negatively correlated with the level of TNF-α in BALF.
