目的探讨1.5 T磁共振扩散张量成像(DTI)对前列腺增生(BPH)及前列腺癌(PCa)的诊断价值。方法搜集昆明医科大学第二附属医院行前列腺常规MRI及DTI扫描,并经病理证实的38例BPH和25例PCa,分析两组的表观扩散系数(ADC)图、分数各向异性(FA)图、纤维示踪图(FT)表现及ADC值、FA值参数特点,对比两组DTI参数有无统计学差异。结果 (1)ADC、FA图两组表现一致,FT的形态不同,纤维束中断见于PCa;(2)BPH组中央叶与外围叶ADC值、FA值均存在差异(P=0.000),PCa组则均不存在差异(P=0.084,0.205),两组间比较中央叶DTI参数不存在差异(P=1.000),外围叶存在差异(P=0.000)。结论 DTI能反映常见前列腺疾病的微观结构变化,可以辅助常规MRI诊断前列腺疾病。
Objective To investigate the diagnostic value of 1.5 T magnetic resonance diffusion tensor imaging (DTI) in benign prostatic hyperplasia (BPH) and prostate cancer (PCa). Methods The routine ADC and DTI scan of prostate in the Second Affiliated Hospital of Kunming Medical University were collected. 38 cases of BPH and 25 cases of PCa were confirmed by pathology. The apparent diffusion coefficient (ADC), fractional anisotropy (FA) Figure, fibrin trace (FT) performance and ADC value, FA value characteristics of the parameters, compared with two groups of DTI parameters have no statistical difference. Results (1) ADC and FA showed the same appearance in both groups, with different FT morphology and fiber bundle disruption in PCa. (2) There were significant differences in ADC value and FA value between central lobe and peripheral lobe in BPH group (P = 0.000) There was no difference (P = 0.084,0.205). There was no difference in central DTI parameters between the two groups (P = 1.000) and peripheral leaf (P = 0.000). Conclusion DTI can reflect the microscopic structure changes of common prostate diseases and can help the diagnosis of prostate diseases by conventional MRI.
