目的:探讨结合多种门控技术克服呼吸运动和心脏搏动对肝脏磁共振扩散加权成像(DWI)的影响。方法:选择2016年1月至2016年12月中青年健康志愿者50例,均行无门控、多种门控技术(如呼吸门控、脉搏门控和心电门控等)进行肝脏DWI检查,其中无门控组检测时视为对照组,实施多种门控组时视为观察组,b值选取0和500 s·mm-2,分别采用不同的延迟触发时间触发扫描。第1次肝脏DWI扫描为10点和19点,采集2次的ADC值,由2名测量者分别测量,测量者1测完第1次数据后,间隔15 d再进行第2次测量,总结分析ADC值,得出结论。结果:对照组肝左叶ADC值明显高于肝右叶;观察组肝左叶ADC值明显低于肝右叶;Bland–Altman分析显示,该方法可重复率高;延迟触发时间为500 ms时,可重复率高。结论:多种门控技术能有效降低ADC值,提高肝脏DWI扫描图像质量,可重复性高,最佳触发时间为500 ms。采用多种门控技术克服心脏搏动效果比较理想。
OBJECTIVE: To explore the effect of combining multiple gating techniques on overcoming diffusion-weighted imaging (DWI) of liver by respiratory motion and heart beating. Methods: From January 2016 to December 2016, 50 healthy young volunteers were enrolled in this study. All patients underwent portal-free and multi-gating techniques (such as respiratory gating, pulse gating and ECG gating) The control group was regarded as the control group when there was no gated control group, and the observation group was regarded as the observation group when multiple gated groups were implemented. The values of b were selected as 0 and 500 s · mm-2, and different delayed triggering time-triggered scans were used respectively. The first liver DWI scan at 10 o’clock and 19 o’clock, the ADC value was collected twice, measured by two surveyors, measuring 1 1 after the completion of the first data, the interval of 15 d and then the second measurement summary Analyze the ADC values and conclude. Results: The ADC value of the left lobe of the control group was significantly higher than that of the right lobe. CONCLUSION: A variety of gating techniques can effectively reduce the ADC value and improve the quality of liver DWI scan images with high repeatability and the best triggering time is 500 ms. Using a variety of gating technology to overcome the heart beat effect is ideal.
