心肌梗塞多由冠心病引起,但也可能由特定的病因引起,近年已见有这方面的报道。现将我们近2年中所遇的非冠心病所致的少见心肌梗塞2例报告如下,以引起同道的重视。例1:男,34岁。因突发剧烈头痛呕吐伴昏迷半天而入院。体检:神志不清,颈强直,心肺听诊正常。脑脊液呈均匀鲜红色,压力高。以往健康,喜好体育锻炼,无高血压及心脏病病史,亦无冠心病易患因素存在。入院后第2天EKG示广泛前壁心肌梗塞。经积极治疗后心肌梗塞图形持续1周后渐消失,但终因脑出血量大,昏迷时间长,于病后10天死亡。临床诊断为重症蛛网膜下腔出血(SAH)合并广泛前壁心肌梗塞。

Physical examination: unconscious, neck stiffness, cardiopulmonary auscultation normal. Cerebrospinal fluid was bright red, high pressure. Past health, like physical exercise, no history of hypertension and heart disease, there is no risk factors for coronary heart disease exist. On the second day after admission, EKG showed extensive anterior myocardial infarction. After active treatment, myocardial infarction pattern continued to disappear after 1 week, but the end result of cerebral hemorrhage, coma for a long time, died in 10 days after the disease. Clinical diagnosis of severe subarachnoid hemorrhage (SAH) with extensive anterior myocardial infarction.
