目的评价多西他赛(希存,DOC)联合顺铂(DDP)、氟尿嘧啶(5-Fu)治疗晚期远处转移鼻咽癌的初步疗效及其毒副反应。方法治疗晚期远处转移鼻咽癌患者32例,给予DOC 75 mg/m2及DDP 75 mg/m2静脉滴注,第1天;5-Fu 500 mg/m2,第1～5天持续静脉灌注;每3周为1周期,所有患者均治疗2～4周期,观察毒副反应并评价化疗疗效。结果CR 5例(15.6%),PR 18例(56.3%),SD 7例(21.9%),PD 2例(6.3%)。总有效率72.0%。该方案最主要的毒副反应为骨髓抑制,Ⅲ～Ⅳ度粒细胞减少占68.8%;Ⅱ～Ⅲ度血小板下降占50.0%,其余毒副反应则可以耐受。结论多西他赛(希存)联合顺铂、氟尿嘧啶治疗晚期远处转移鼻咽癌的近期临床疗效较好,毒副反应可以耐受,值得临床推广。
Objective To evaluate the preliminary efficacy and toxicity of docetaxel combined with cisplatin (DDP) and 5-fluorouracil in the treatment of advanced distant metastasis of nasopharyngeal carcinoma. Methods Thirty-two patients with advanced nasopharyngeal carcinoma were treated by intravenous infusion of DOC 75 mg / m2 and DDP 75 mg / m2 on day 1; 5-Fu 500 mg / m2 on day 1 to 5; Every 3 weeks for a cycle, all patients were treated for 2 to 4 cycles to observe the side effects and evaluate the efficacy of chemotherapy. Results CR 5 cases (15.6%), PR 18 cases (56.3%), SD 7 cases (21.9%) and PD 2 cases (6.3%). The total effective rate of 72.0%. The most important side effects of this regimen are myelosuppression, with Ⅲ ~ Ⅳ neutropenia accounting for 68.8%, Ⅱ ~ Ⅲ degree platelet declining 50.0%, and the rest of the toxicity can be tolerated. Conclusion Docetaxel combined with cisplatin and fluorouracil in the treatment of advanced distant metastasis of nasopharyngeal carcinoma has good clinical efficacy and tolerability of toxic and side effects, which is worthy of clinical promotion.
