目的:探讨超声乳化联合TECNIS散光矫正型(Toric)人工晶状体(IOL)植入术治疗白内障的效果。方法:抽取2020年1月至2021年6月周口市眼科医院收治的90例(98眼)角膜散光≥0.75 D的白内障患者,随机分为甲组(45例51眼)、乙组(45例47眼),两组均行超声乳化联合IOL植入术,甲、乙组分别植入TECNIS非球面多焦点Toric IOL ZMA000、TECNIS非球面单焦点IOL,比较两组术前、术后1周、术后3个月裸眼远视力(UDVA)、裸眼近视力(UNVA)、最佳矫正近视力(BCNVA)、最佳矫正远视力(BCDVA)、残余散光度,术后3个月不同空间频率对比敏感度(CS)及视觉质量指标客观散射指数(OSI)、斯特利尔比值(SR)、调制传递函数(MTF)截止空间频率的变化,统计术后3个月脱镜率及视觉干扰情况。结果:甲组术后1周、3个月UNVA、BCNVA优于乙组(n P<0.05);甲组术后1周、术后3个月残余散光度低于乙组(n P<0.05);甲组18 c/d空间频率下CS高于乙组(n P<0.05);甲组术后3个月OSI、SR、MTF截止空间频率均高于乙组(n P<0.05);甲组术后3个月脱镜率高于乙组(n P0.05。n 结论:超声乳化联合TECNIS非球面多焦点Toric IOL ZMA000植入可促进白内障患者远近视力改善,优化全程视力,减少残余散光,且术后早期视觉质量高,CS好,脱镜率高。“,”Objective:To investigate the effect of phacoemulsification combined with TECNIS toric intraocular lens (IOL) implantation in the treatment of cataract.Methods:A total of 90 patients (98 eyes) with cataract and corneal astigmatism no smaller than 0.75 D who were admitted to Zhoukou Eye Hospital between January 2020 and June 2021 were randomly, and divided into group A (45 cases, 51 eyes) and group B (45 cases, 47 eyes). Both groups were treated with phacoemulsification and IOL implantation. TECNIS aspherical multifocal Toric IOL ZMA000 and TECNIS aspherical monofocal IOL were used in group A and group B respectively. Comparison was made between the two groups in terms of the uncorrected distance visual acuity (UDVA), uncorrected near visual acuity (UNVA), best corrected near visual acuity (BCNVA), best corrected distance visual acuity (BCDVA) and residual astigmatism before operation and at 1 week and 3 months after operation, the contrast sensitivity (CS) of different spatial frequencies, visual quality index objective scatter index (OSI), Strehl ratio (SR) and cut-off spatial frequency of modulation transfer function (MTF) at 3 months after operation. The spectacles-independent rate and visual disturbance at 3 months after operation were statistically analyzed.Results:UNVA and BCNVA in group a were better than those in group B at 1 week and 3 months after operation (n P<0.05). The residual astigmatism in group A was lower than that in group B at 1 week and 3 months after operation (n P<0.05). The CS under 18 c/d spatial frequency of group A was higher than that of group B (n P<0.05). The OSI, SR and the cut-off spatial frequency of MTF in group A were higher than those in group B at 3 months after operation (n P<0.05). The spectacles-independent rate in group A was higher than that in group B at 3 months after operation (n P0.05).n Conclusions:Phacoemulsification combined with TECNIS aspheric multifocal Toric IOL ZMA000 implantation can promote the improvement of distant and near visual acuity of cataract patients, optimize full range of vision, and reduce residual astigmatism. And the visual quality is high, CS is good and the spectacles-independent rate is high in early stage after operation.
