目的检测妊娠期糖尿病(gestational diabetes mellitus,GDM)患者不同孕龄血清胱抑素C(cystatin C,CysC)及血清尿酸(serum uric acid,UA)水平变化,探讨两者在监测GDM患者发生妊娠期高血压疾病(hypertensive disorder of pregnancy,HDP)的早期病情变化的临床意义。方法选取2010年1月至2012年1月于南海区妇幼保健院进行定期产前检查确诊为GDM的患者114例,将其中妊娠晚期合并HDP患者23例设为A组,无并发症91例设为B组,选取同期90例正常妊娠孕妇为C组,将3组孕妇在孕24~28周、32~34周、38~40周的Cys C、UA检测结果进行比较。结果①3组不同孕龄的血清Cys C比较,差异有统计学意义(F=106.215,P<0.05),且组别与孕龄间存在交互作用(F=26.300,P<0.05),不同孕龄Cys C的检测结果可以用二次方曲线拟合;②3组不同孕龄血清UA检测结果差异有统计学意义(F=21.08,P<0.05),且组别与孕龄间存在交互作用(F=4.11,P<0.05),不同孕龄血清UA的检测结果符合直线趋势拟合。结论血清Cys C与血清UA均与GDM的病情相关,血清CysC联合血清UA的测定在预测GDM合并HDP和肾功能早期损伤有重要意义。
Objective To detect serum cystatin C (CysC) and serum uric acid (UA) in patients with gestational diabetes mellitus (gestational diabetes mellitus, gestational age, gestational age, Clinical Significance of Early Changes of Hypertensive Disorder of Pregnancy (HDP). Methods From January 2010 to January 2012 in Nanhai District Maternal and Child Health Hospital were scheduled for prenatal diagnosis of GDM patients 114 cases, of whom 23 cases of HDCP in late pregnancy as a group A, 91 cases without complications In group B, 90 pregnant women of the same period were selected as group C. The results of Cys C and UA were compared between 24 to 28 weeks, 32 to 34 weeks and 38 to 40 weeks of pregnancy. Results ① There were significant differences in serum Cys C between groups at different gestational ages (F = 106.215, P <0.05), and there was interaction between groups and gestational age (F = 26.300, P <0.05) The test results of Cys C can be fitted by quadratic curve. ② The UA test results of three groups at different gestational ages showed statistical significance (F = 21.08, P <0.05), and there was interaction between group and gestational age = 4.11, P <0.05). The results of UA at different gestational ages accorded with the linear trend fitting. Conclusions Serum Cys C and serum UA are related to the severity of GDM. The determination of serum CysC and serum UA is of great significance in predicting GDM with HDP and early renal injury.
