目的:观察高强度聚焦超声(HI-FU)联合三维适形放疗(3D-CRT)同步治疗壶腹部周围癌的近期疗效。方法:32例壶腹部周围癌(胰头癌22例、Vater壶腹癌5例、胆总管下端癌3例及十二直肠乳头癌2例),给予HIFU联合3D-CRT同步治疗。结果:32例患者均可评价疗效。肿瘤体积变化CR2例、PR24例、NC 4例、PD2例。1、2年生存率分别为81.2%(26/32)和46.9%(15/32)。中位TTP 14.6个月。临床受益指数93.8%(30/32)。结论:HIFU+3D-CRT方案同步治疗壶腹部周围癌可适当延长生存期,减轻患者痛苦,提高生活质量,无明显的不良反应。
Objective: To observe the short-term curative effect of high-intensity focused ultrasound (HI-FU) combined with three-dimensional conformal radiotherapy (3D-CRT) Methods: Thirty-two cases of periampullary carcinoma (22 cases of pancreatic head cancer, 5 cases of Vater ampulla, 3 cases of common bile duct cancer and 2 cases of papillary duodenal papillae) were treated with HIFU and 3D-CRT simultaneously. Results: All 32 patients can evaluate the curative effect. CR2 tumor volume changes, PR24 cases, NC 4 cases, PD2 cases. The 1-year and 2-year survival rates were 81.2% (26/32) and 46.9% (15/32), respectively. Median TTP 14.6 months. Clinical benefit index 93.8% (30/32). Conclusions: The HIFU + 3D-CRT regimen in the treatment of peri-ampullary carcinoma can prolong survival, reduce patient’s pain and improve quality of life without obvious adverse reactions.
