目的探讨连续血液净化治疗重症急性胰腺炎的临床效果。方法选取重庆市长寿区人民医院2015年1—11月收治的重症急性胰腺炎患者90例,随机将患者分为常规组与血液净化组,各45例。常规组给予常规治疗,血液净化组在常规组基础上予以连续血液净化治疗。比较两组患者机械通气治疗时间、住院时间、治疗效果及肿瘤坏死因子α(TNF-α)、白介素6(IL-6)、白介素8(IL-8)水平。结果血液净化组患者机械通气治疗时间、住院时间短于常规组,治疗总有效率高于对照组(P<0.05)。治疗前两组患者TNF-α、IL-6、IL-8水平比较,差异无统计学意义(P>0.05);治疗后血液净化组患者TNF-α、IL-6、IL-8水平低于对照组(P<0.05)。结论连续血液净化治疗重症急性胰腺炎的临床效果确切,可有效降低患者机体炎性反应,改善患者临床症状,缩短机械通气和治疗时间。
Objective To investigate the clinical effect of continuous blood purification in the treatment of severe acute pancreatitis. Methods Ninety patients with severe acute pancreatitis admitted from January 2015 to January 2015 in Changshou District People’s Hospital of Chongqing were randomly divided into routine group and blood purification group, with 45 cases in each group. The conventional group was given routine treatment, and the blood purification group was treated with continuous blood purification on the routine group. The duration of mechanical ventilation, length of hospital stay, therapeutic effect and the levels of tumor necrosis factor alpha (TNF-α), interleukin 6 (IL-6) and interleukin 8 (IL-8) were compared between the two groups. Results In the blood purification group, the duration of mechanical ventilation and hospital stay were shorter than those of the conventional group, and the total effective rate of treatment was higher than that of the control group (P <0.05). The levels of TNF-α, IL-6 and IL-8 in the two groups before treatment were not significantly different (P> 0.05). The levels of TNF-α, IL-6 and IL-8 in the blood purification group were lower than Control group (P <0.05). Conclusion Continuous blood purification for the treatment of severe acute pancreatitis has the exact clinical effect, which can effectively reduce the patient’s inflammatory response, improve the clinical symptoms and shorten the duration of mechanical ventilation and treatment.
