目的 :研究后巩膜加固术 (PSRD)治疗进行性近视的疗效及机理。方法 :对 2 0例 ( 3 6眼 )进行性近视行改良的Snyder Thompson式PSRO ,手术前后观察裸眼视力、矫正视力、眼底、屈光度、眼轴、闪光视网膜电图 (F -ERG) ,振荡电位 (Ops)。术后随访 1年者 3 6眼 ,5年以上 13眼。结果 :( 1)术后 90 %以上的高度近视患者视力稳定 ,部分患者视力有不同程度的提高。 ( 2 )术后眼轴及屈光度稳定 (P >0 0 5 )。( 3 )F -ERG的a波b波波幅术后较术前有提高 (P <0 0 5 ) ,Ops振幅有非常显著性提高 (P <0 0 0 2 ) ,且均随时间的延长 ,提高更加明显。结论 :后巩膜加固术治疗进行性近视 ,的确起到稳定眼轴及屈光度 ,改善黄斑病变及视网膜血液循环 ,从而提高视功能的作用。
Objective: To study the efficacy and mechanism of posterior scleral reinforcement (PSRD) in the treatment of progressive myopia. Methods: Snyder Thompson type PSRO was retrospectively performed in 20 patients (36 eyes). The visual acuity, corrected visual acuity, fundus, refraction, axial length, flash electrogastrogram (F -ERG) (Ops). The patients were followed up for 36 eyes of 1 year, 13 eyes of 5 years or more. Results: (1) After more than 90% of patients with high myopia visual acuity, some patients have different degrees of visual acuity improved. (2) Postoperative axial and refractive stability (P> 0.05). (3) The amplitudes of a-wave and b-wave of F -ERG increased significantly (P <0.05), and the amplitude of Ops increased significantly (P <0 0 0 2) Improve more clearly. Conclusion: Posterior scleral reinforcement for the treatment of progressive myopia, does play a stable axial and refractive, improve macular degeneration and retinal blood circulation, thereby enhancing the role of visual function.
