目的探讨白介素-11(IL-11)及可溶性糖蛋白130(sgp130)在初诊急性白血病(AL)诱导缓解过程中的变化及临床意义,研究基因重组人白介素-11(rhIL-11)对AL患者血小板恢复的影响。方法将苏州大学附属第一医院和徐州医学院第二附属医院血液科2004年4月至2005年10月收治的64例初诊AL患者分为A、B两组,采用ELISA检测方法对初诊AL患者(A组)化疗前及获完全缓解(CR)后的血清IL-11及sgp130质量浓度进行动态观测;将rhIL-11应用于初诊AL患者(B组)。结果AL患者血清IL-11明显低于正常人(P<0.05),经诱导化疗获CR后明显升高至正常水平,且与外周血血小板数呈平行性升高或降低;sgp130明显高于正常人(P<0.05),获CR后明显下降至正常水平,且与外周血白细胞数呈正相关。注射rhIL-11后血小板恢复至≥30×109/L、≥50×109/L、≥100×109/L的平均时间均早于未使用rhIL-11者,且血小板输注量明显减少。结论检测AL患者血清IL-11及sgp130水平可作为了解AL患者病情、判断疗效和预后的有意义的指标;rhIL-11可以有效地促进AL患者诱导缓解化疗后外周血小板的恢复。
Objective To investigate the changes and clinical significance of interleukin-11 (IL-11) and soluble glycoprotein 130 (sgp130) in the induction of acute leukemia (AL) -induced remission and to investigate the effect of recombinant human interleukin-11 (rhIL- Effects of platelet recovery. Methods 64 cases of newly diagnosed AL patients admitted to the First Affiliated Hospital of Soochow University and the Second Affiliated Hospital of Xuzhou Medical College from April 2004 to October 2005 were divided into A and B groups. (Group A) before and after complete remission (CR) serum IL-11 and sgp130 concentration dynamic observation; rhIL-11 was applied to newly diagnosed AL patients (group B). Results The serum level of IL-11 in patients with AL was significantly lower than that in normal people (P <0.05), and the level of IL-11 in AL patients was significantly increased to normal level after CR and increased or decreased in parallel with peripheral blood platelets; sgp130 was significantly higher than normal (P <0.05), and then decreased to normal level after CR, which was positively correlated with the number of leukocytes in peripheral blood. The mean time to platelet recovery to ≥30 × 109 / L, ≥50 × 109 / L, ≥100 × 109 / L after rhIL-11 injection was earlier than that of non-use of rhIL-11, and the platelet transfusion volume was significantly reduced. Conclusion Detection of serum IL-11 and sgp130 levels in patients with AL can be used as a meaningful indicator for understanding the prognosis and prognosis of AL patients. RhIL-11 can effectively promote the recovery of peripheral platelets induced by AL after chemotherapy.
