目的探讨300mgI/ml对比剂与400mgI/ml对比剂对肾脏多层面CT(multislice CT,MSCT)多期增强扫描的强化作用。方法60例疑肾脏病变的患者随机分成两组各20例,分别给予肾脏平扫和典比乐300(300mgI/ml)与碘迈伦400(400mgI/ml)的多期增强MSCT扫描。采用Toshiba Aquil-ion64扫描仪,在对比剂开始注射后18s、30s、80s、3~5min扫描。测量各期增强扫描腹主动脉、双肾动脉、双肾静脉、双肾皮质、双肾髓质、双肾盂、肾脏病变的CT强化值。观察上述结构的强化情况。结果使用400mgI/ml对比剂在18s与30s采集,所检测的血管与肾各结构强化均值有意义高于300mgI/ml对比剂(P<0.01),80s采集,肾动脉、肾静脉、肾髓质强化均值有意义高于300mgI/ml对比剂(P<0.01),3~5min采集,肾静脉与肾盂强化均值有意义高于300mgI/ml对比剂(P<0.01)。结论高碘浓度对比剂对肾血管的显示优于标准碘浓度对比剂,并可降低对比剂用量。肾脏MSCT高碘浓度对比剂增强扫描在对比剂开始注射后30s与延时3~5min采集较合理。
Objective To investigate the potentiating effect of 300mgI / ml contrast agent and 400mgI / ml contrast agent on multi-phase enhanced CT in multislice CT (MSCT) of the kidney. Methods Sixty patients with suspected renal disease were randomly divided into two groups of 20 patients. MS and MSCT of renal plain and dystopian 300 (300mgI / ml) and 400mgI / ml were given respectively. The Toshiba Aquil-ion 64 scanner was used to scan 18s, 30s, 80s and 3 ~ 5min after contrast injection began. The CT enhancement value of enhanced abdominal aorta, renal artery, renal vein, renal cortex, double renal medulla, double renal pelvis and kidney lesions in each stage was measured. Observe the strengthening of the above structure. Results 400mgI / ml contrast agent was collected in 18s and 30s. The mean value of structural enhancement of blood vessels and kidneys was significantly higher than that of 300mgI / ml contrast agent (P <0.01). Renal artery, renal vein, renal medulla The mean value of enhancement was significantly higher than that of 300mgI / ml contrast agent (P <0.01), and the mean value of renal vein and pyelography was significantly higher than that of 300mgI / ml contrast agent (P <0.01). Conclusion High iodine concentration contrast agent shows superiority of renal blood vessels compared with standard iodine contrast agent, and can reduce the contrast agent dosage. Renal MSCT high iodine concentration contrast enhanced contrast acquisition 30s after the start of the contrast agent with a delay of 3 ~ 5min collection more reasonable.
