目的研究不同年龄组小儿血浆靶控丙泊酚的最佳靶控浓度。方法将1-12岁的小儿,按年龄将其分为3组,Ⅰ组(1-3岁)、Ⅱ组(3-6岁)、Ⅲ组(6-12岁)每组30人共计90人。每个年龄组的患儿起始血浆靶浓度为3μg/ml(先通过预实验而设置的起始靶浓度),采用改良的Dixon’s上下调整法确定各年龄组患儿的最佳靶控浓度(每次增减0.2μg/ml),若上一例达到标准,则下1例采用低一级浓度;反之,若上一例未达到标准,则下1例采用高一级浓度;并且连续监测ECG、HR、BP、SPO2、RR、BIS值。结果Ⅰ组血浆靶控丙泊酚的最佳靶控浓度的ED50及其95%可信区间为3.34(2.61-4.19)μg/ml;Ⅱ组血浆靶控丙泊酚的最佳靶控浓度的ED50及其95%可信区间为3.05(2.52-3.64)μg/ml;Ⅲ组血浆靶控丙泊酚的最佳靶控浓度的ED50及其95%可信区间为2.92(2.67-3.56)μg/ml。结论不同年龄组的患儿最佳血浆靶控丙泊酚浓度不同,年龄小的血浆靶控丙泊酚的最佳靶控浓度的ED50较大。
Objective To study the optimal target control concentration of propofol target plasma in children of different age groups. Methods Children aged 1-12 years were divided into three groups according to their age: Group Ⅰ (1-3 years), Group Ⅱ (3-6 years), Group Ⅲ (6-12 years) people. The starting plasma target concentration for each age group was 3 μg / ml (initial target concentration set by preliminary experiments), and the optimal Dixon’s up-and-down adjustment method was used to determine the optimal target concentration for each age group ( Each increase or decrease 0.2μg / ml), if the previous case reached the standard, the next one case of low concentration; the other hand, if the previous case did not meet the standard, then the next one case of a higher concentration; and continuous monitoring of ECG, HR, BP, SPO2, RR, BIS values. Results The optimal ED50 and 95% confidence interval of plasma target propofol in group Ⅰ were 3.34 (2.61-4.19) μg / ml. The best target control concentration The ED50 and its 95% confidence interval were 3.05 (2.52-3.64) μg / ml respectively. The ED50 and 95% confidence interval (ED50) of plasma target propofol concentration in group Ⅲ were 2.92 (2.67-3.56) μg / ml. Conclusion The best target plasma propofol concentrations in children of different age groups are different, and the ED50 of the target propofol target plasma with smaller age is larger.
