双胎新生儿的临床问题探讨山西医学院（０３０００１）张小莉山西省儿童医院张香莲现将山西省儿童医院新生儿病区二年来收治的双胎新生儿的患病和死亡情况进行初步分析。临床资料１．一般资料：双胎新生儿４１对８２例，其中患病者５５例（包括１１对２２例孪生于双双入院），死胎１例，死产２例，健康者２４例。在５５例患病者中，早产４０例，足月产１５例；出生体重＜２５００ｓ者４６例，其中＜１５００ｇ者６例，＞２５００ｇ者９例；５５例中死亡１８例，死亡率３２。７３％，远远高于同期住院的单胎新生儿的死亡率。２．患病情况：多数患有一种以上疾病，其中以各种感染最多见占３３例，其次为硬肿症２３例、颅内出血１９例、肺透明膜病５例、贫血４例和维生素Ｅ缺乏症１例。在３３例感染性疾病中，败血症１４例，化脓性脑膜炎３例，肺炎１１例，脐部和／或皮肤感染５例。３死亡原因：硬肿症合并肺出血９例，严重感染引起的循环和呼吸衰竭３例，肺透明膜病引起的呼吸衰竭３例，严重颅内出血２例，极低出生体重儿中枢性呼吸衰竭伴循环衰竭１例。讨论我国双胎分娩的机会稍少，据报道约１００次分娩中有１例［１］。双胎新生儿普遍体重低，营养发育差，成熟度低，患病率和病死率高。本组显示出危害
